三叉神经痛合并青光眼患者经皮穿刺球囊压迫术中出现严重三叉神经-心脏反射:1例报道与文献复习

摘　　要：三叉神经-心脏反射是经皮穿刺球囊压迫术治疗三叉神经痛术中常见并发症, 术前或术中应用阿托品可有效预防心动过缓和心脏停搏的发生。本文报道1例阿托品使用禁忌的三叉神经痛合并青光眼患者, 球囊压迫术中出现严重、频发心动过缓。采用小剂量、间断、缓慢增加球囊容量压迫三叉神经节, 安全、顺利地完成手术。术毕患者疼痛消失, 未见严重并发症, 术后随访6个月疼痛未复发。Trigeminocardiac reflex is a common complication during percutaneous balloon compression for treatment of trigeminal neuralgia.Atropine pre-or intra-operatively can prevent the occurrence of bradycardia or cardiac arrest effectively.This paper reported that one patient with glaucoma contraindicated to atropine and occurred a severe and frequent bradycardia during percutaneous balloon compression therapy,and that the operation was complete successfully by small dose,intermittent and slowly increasing balloon volume.The pain was relieved immediately and no serious adverse reactions happened.
