摘　　要：培门冬酶是大肠杆菌来源的左旋门冬酰胺酶与聚乙二醇(PEG)及磷脂双分子层连接形成的一种长效门冬酰胺酶。相比左旋门冬酰胺酶,培门冬酶半衰期更长,免疫原性更低。2009年国家药品监督管理局(NMPA)批准培门冬酶用于儿童急性淋巴细胞白血病患者的一线治疗。培门冬酶自上市以来,在儿童和成年人急性淋巴细胞白血病以及恶性淋巴瘤中均广泛使用。为指导临床规范化应用培门冬酶,使患者最大限度获益,中华医学会血液学分会专家委员会组织多学科专家,讨论了培门冬酶的临床应用及不良反应的处理措施,形成了本共识,供临床医师参考。Pegaspargase,a kind of long-acting asparaginase,is a pegylated form of Escherichia coli-derived L-asparaginase,in which L-asparaginase is covalently linked to polyethylene glycol(PEG)and phospholipid bilayer.Compared with L-asparaginase,pegaspargase has longer half-life period and lower immunogenicity.In 2009,it was approved by the National Medical Products Administration(NMPA)for the first-line treatment of children with acute lymphoblastic leukemia(ALL).Since its launch,pegaspargase has been widely used in the treatment of ALL and malignant lymphoma in both children and adults.In order to guide the standardized clinical application of pegaspargase and maximize the clinical benefits of patients,the Expert Committee of the Chinese Society of Hematology organized multidisciplinary experts to discuss its clinical application and the management of toxicities,and the following guiding principles were developed for clinicians'reference.
