幽门螺杆菌感染与儿童电子胃镜检查结果的临床分析

摘　　要：目的探讨儿童上消化道疾病的临床特点,幽门螺杆菌(Hp)感染与胃及十二指肠黏膜病变的关系。方法回顾性分析802名患儿既接受电子胃镜检查又进行Hp的检测,使用倾向评分匹配分析(PSMA)对2组患儿进行了回顾性评估。2组患儿使用1∶1及PS得分为0.02以内的就近匹配原则。经PSMA后产生206个匹配对。结果406例患儿中,非Hp感染组年龄(8.37±1.23)岁、Hp感染组年龄(10.63±2.21)岁,随着年龄增长,Hp感染率增加。Hp感染组以呕血、黑便起病的发生率明显高于非Hp感染组(P<0.05)。Hp感染组在胃镜检查结果中浅表性胃炎、浅表性胃炎伴糜烂、胃(窦)溃疡、十二指肠溃疡的发生率明显高于非Hp感染组。结论Hp感染率与年龄呈正相关关系,儿童胃十二指肠疾病与Hp感染密切相关,尤其以胃、十二指肠溃疡与Hp感染关系更为密切。Objective To explore the clinical features of upper gastrointestinal diseases in children,and to explore the relationship between Helicobacter pylori(Hp)infection and gastric and duodenal mucosal lesions.Methods A total of 802 children underwent both electronic gastroscopy and Hp test in a retrospective analysis.The propensity score matching analysis(PSMA)was used to retrospectively evaluate the two groups of children.PSMA was used for multivariate logistic regression to complete the model based on:age,gender,inspection season,residential area,state at the time of inspection(normal,painless).For the two groups of patients,the nearest matching principle of 1∶1 and PS score within 0.02 was used.After PSMA,206 matching pairs were generated.Results Among the 406 children,the age of the non-Hp infection group was(8.37±1.23)years old,and the age of the Hp infection group was(10.63±2.21)years old,suggesting that with age,the Hp infection rate increased.The incidence of onset of hematemesis and melena in the Hp infection group was significantly higher than that in the non-Hp infection group,and the differences were statistically significant.The incidence of superficial gastritis,superficial gastritis with erosion,gastric(antral)ulcer,and duodenal ulcer in the HP infection group was significantly higher than that of the non-Hp infection group.Conclusion Hp infection rate is positively correlated with age.Children's gastroduodenal diseases are closely related to Hp infection,especially gastric and duodenal ulcers are more closely related to Hp infection.
