摘  要:目的 探究高分辨率超声(HRUS)与数字化彩色超声在胆囊腺肌增生症中的诊断效能。方法 选取2018年9月至2021年8月天津市三潭医院收治的70例疑似胆囊腺肌增生症患者为研究对象,均接受HRUS、数字化超声检查及联合检查,以病理诊断结果为金标准,比较HRUS、数字化超声检查及联合检查在胆囊腺肌增生症中的诊断效能。结果 病理诊断结果显示,70例疑似胆囊腺肌增生症患者中阳性43例,阴性27例;HRUS检查结果显示,70例疑似胆囊腺肌增生症患者中阳性38例,阴性32例;数字化彩色超声检查结果显示,70例疑似胆囊腺肌增生症患者中阳性37例,阴性33例;联合检查结果显示,70例疑似胆囊腺肌增生症患者中阳性43例,阴性27例。以病理结果为金标准,联合检查诊断胆囊腺肌增生症的灵敏度、准确度均高于HRUS、数字化超声检查,漏诊率低于HRUS、数字化超声检查,差异有统计学意义(P <0.05)。HRUS、数字化彩色超声检查及联合检查诊断胆囊腺肌增生症的ROC曲线下面积(AUC)分别为0.730、0.712、0.856(P <0.05)。结论 HRUS、数字化超声联合检查能够提升诊断胆囊腺肌增生症的灵敏度和准确度,降低漏诊率,建议临床选择联合检查对胆囊腺肌增生症实施评估诊断。Objective The diagnostic efficacy of high-resolution ultrasound(HRUS)and digital color ultrasound in gallbladder adenomyosis was explored.Methods With 70 patients with suspected gallbladder adenomyosis admitted to Tianjin Santan Hospital from September 2018 to August 2021 as the research subjects,HRUS,digital ultrasonic detection and combined detection were performed on them.The diagnostic efficacy of HRUS,digital ultrasound,and combined detection in gallbladder adenomyosis was compared based on pathological diagnosis results as the gold standard.Results The results of pathological diagnosis showed that 43 cases were positive and 27 cases were negative in 70 patients with suspected gallbladder adenomyosis;The results of HRUS test showed that 38 cases were positive and 32 cases were negative in 70 patients with suspected gallbladder adenomyosis;The results of digital color ultrasound showed that 37 cases were positive and 33 cases were negative in 70 patients with suspected gallbladder adenomyosis;The combined test results showed that 43 cases were positive and 27 cases were negative in 70 suspected patients with gallbladder adenomyosis.Based on pathological results as the gold standard,the sensitivity and accuracy of combined detection in the diagnosis of gallbladder adenomyosis were higher than those of HRUS and digital ultrasound detection,while the missed diagnosis rate was lower than that of HRUS and digital ultrasound detection,with statistical significance(P<0.05).The area under the ROC curve(AUC)of HRUS,digital color ultrasound,and combined detection for the diagnosis of gallbladder adenomyosis were 0.730,0.712,0.856,respectively(P<0.05).Conclusions The combined detection of HRUS and digital ultrasound can improve the sensitivity and accuracy of diagnosing gallbladder adenomyosis,and reduce the rate of missed diagnosis.It is recommended to choose a combined detection for clinical evaluation and diagnosis of gallbladder adenomyosis.
