牙周炎对老年男性冠心病患病及发病的影响

摘　　要：目的探讨牙周炎对老年男性冠心病(CHD)患病和CHD发病的影响。方法选择2019年5月—2022年7月在商丘市中医院体检的老年男性1534人作为研究对象。以体检日期作为研究基线。首先进行横断面调查,对调查对象进行牙周炎和CHD的筛查,通过问卷调查确定调查对象基线资料。根据调查对象CHD患病状况将其分为CHD组和非CHD组,应用多因素分析探讨老年男性CHD患病的影响因素,以确定牙周炎和CHD患病的关联。然后对未患有CHD的调查对象进行随访,根据调查对象后期CHD发病状况将其分为CHD组和非CHD组,应用多因素分析探讨老年男性CHD发病的影响因素,以确定牙周炎和CHD发病的关联。结果(1)年龄大、高血压、糖尿病、高血脂症、吸烟、牙周炎是老年男性CHD患病的危险影响因素,P<0.05,OR分别为1.273、2.154、3.082、3.146、1.325、1.593;(2)糖尿病、慢性阻塞性肺疾病、身体质量指数高、高敏C反应蛋白水平高、CHD家族史、吸烟是老年男性CHD发病的危险因素,P<0.05,OR分别为4.025、1.894、1.319、2.213、1.997、1.452。结论在有牙齿的老年男性人群中,牙周炎和患者CHD患病率相关;牙周炎和患者CHD发病没有关联。Objective To investigate the effect of periodontitis on the prevalence and incidence of CHD in elderly men.Methods Totally 1534 healthy elderly men were selected as the subjects.The date of physical examination was used as the study baseline.Firstly,the cross-sectional survey was conducted to screen the respondents for periodontitis and CHD,and the baseline data of the respondents were determined through questionnaire survey.According to the prevalence of CHD,the subjects were divided into CHD group and non CHD group.Multivariate analysis was used to explore the influencing factors of CHD in elderly men,so as to determine the relationship between periodontitis and CHD.Then,the respondents who did not suffer from CHD were followed up.According to the incidence of CHD in the later stage,they were divided into CHD group and non CHD group.Multivariate analysis was used to explore the influencing factors of CHD in elderly men,so as to determine the relationship between periodontitis and CHD.Results Age,hypertension,diabetes,hyperlipidemia,smoking and periodontitis were the risk factors for CHD prevalence in middle-aged and elderly men.P<0.05,0R were 1.273,2.154,3.082,3.146,1.325 and 1.593 respectively.②Diabetes,chronic obstructive pulmonary disease,high body mass index,high level of C reactive protein,CHD family history and smoking were the risk factors of CHD in middle-aged and elderly men.P<0.05,OR were 4.025,1.894,1.319,2.213,1.997,1.452,respectively.Conclusion Periodontitis is associated with the prevalence of CHD in elderly men with teeth;There was no association between periodontitis and CHD.
