摘 要:目的分析从粪便筛查分离出的产NDM-1型碳青霉烯酶的豚鼠气单胞菌(XX182)的抗菌药物耐药表型和基因组特征,阐明该菌株的抗菌药物耐药机制及其耐药性转移的能力。方法肉汤微量稀释法检测该菌株的抗菌药物敏感性,PCR检测碳青霉烯酶耐药基因亚型(bla_(NDM)、bla_(VIM)、bla_(IMP)、bla_(KPC)),接合试验检测耐药基因的可转移性,通过全基因组测序(WGS)和生物信息学分析明确该菌株的基因组特征。结果该菌株对头孢菌素、碳青霉烯类、环丙沙星和多黏菌素耐药,对替加环素、阿米卡星和氨曲南敏感。该菌株携带多种抗菌药物耐药基因,主要包括bla_(NDM-1)、bla_(MOX-6)、bla_(RSA-1)、bla_(TEM-1)、aac(3)-Ⅱd、cmlA5、arr-2等。毒力因子分析发现其携带有极鞭毛、侧鞭毛、Ⅵ型分泌系统(T6SS)和溶血素A等毒力因子。结论该豚鼠气单胞菌对碳青霉烯类抗菌药物耐药主要由产NDM-1型碳青霉烯酶导致,且bla_(NDM-1)耐药基因位于不可接合质粒上。尽管目前气单胞菌属对碳青霉烯类耐药率较低,但仍应加强对该克隆菌株的监测。Objective To analyze the antibiotic resistantce phenotype and genomic characteristics of a strain of NDM-1-producing Aeromonas caviae(A.caviae,XX182)isolated from stool specimens and elucidate the mechanism of antibiotic resistance and its ability of drug-resistance transfer.Methods The minimal inhibitory concentrations(MICs)were determined by using broth microdilution method.The gene subtype bla_(NDM),bla_(VIM),bla_(IMP) and bla_(KPC),were detected by PCR amplification and genomic features were analyzed by whole-genome sequencing(WGS)and bioinformatics.Results The isolated strain was multidrug resistant to cephalosporins,carbapenems,ciprofloxacin and polymyxin,but remained susceptible to tigecycline,amikacin and aztreonam.Genomic analysis suggested the strain carried a variety of antimicrobial resistance genes,including bla_(NDM-1),bla_(MOX-6),bla_(RSA-1),bla_(TEM-1),aac(3)-Ⅱd,cmlA5 and arr-2.The analysis for the carrying virulence factors found lateral flagella,polar flagella,the typeⅥsecretion system(T6SS)and hemolysin A.Conclusion Carbapenem resistance of the isolated A.caviae was primarily related to the production of NDM-1 carbapenemase,and the drug-resistant gene bla_(NDM-1)located on a non-typable and non-conjugable plasmid.Although currently the overall resistance rate of Aeromonas to carbapenems is relatively low,the surveillance of the clones producing carbapenemase should be strengthened.
