摘 要:目的利用360°水平方位8声源测试设备,初步探讨低中频听力正常老年人水平声源定位特点及影响因素,为听障老年人的听力康复评估提供参考。方法收集本中心招募的年龄≥60岁的低中频(0.25-2kHz)听力正常的老年受试者30人为观察组,经简易智能精神量表(Mini-mental state examination,MMSE)评估认知状态良好,同时选取30名听力正常的中青年受试者(20-40岁)为对照组,行纯音听阈测试和声源定位(Sound localization,SL)能力评估。SL评估采用360°8声源(间隔45°)测试,给声刺激分别为0.25、1和4kHz的啭音,通过计算平均绝对误差(Mean absolutely error,MAE)和反应时长来评估受试者的声源定位能力。结果①老年组0.25、1和4kHz的MAE分别(30.62±13.61)°、(31.51±14.71)°和(31.47±14.22)°;对照组0.25、1和4kHz的MAE分别(20.66±10.05)°、(22.37±9.79)°和(22.09±9.59)°。相同频率组间比较差异均有统计学意义(P=0.002,0.006,0.004)。②不同频率MAE对比:老年组和对照组均为250Hz时MAE最小,其次为4kHz,1kHz最大。③不同方位的MAE比较:老年组和对照组均为0°MAE最大,180°次之,±90°MAE最小。④老年组的反应时长大于对照组,但没有统计学差异。结论①低中频听力正常老年组整圆水平SL能力相比健听的中青年组有所下降。②SL能力与声源频率和声源方位有关。Objective To report characteristics of 360°horizontal low and medium frequency sound sources localization in the normal hearing elderly and influencing factors,to facilitate hearing rehabilitation evaluation in elder pa‐tients with hearing impairment.Methods Thirty elder subjects(aged 60 years and over)with normal hearing at low and medium frequencies(0.25-2 kHz)were included after cognitive status assessment using the mini mental state examination(MMSE).Thirty young and middle aged subjects(20-40 years old)with normal hearing were selected as the control group.Pure tone hearing thresholds were evaluated.Sound localization(SL)ability was evaluated using warble sound at 0.25,1 and 4 kHz from 8 sound sources(at 45°intervals)and by calculating mean absolute error(MAE)and response time.Results The MAE.The MAE were 30.62±13.61,31.51±14.71 and 31.47±14.22 degrees in the elderly group,and 20.66±10.05,22.37±9.79 and 22.09±9.59 degrees in the control group for 0.25,1 and 4 kHz,respectively(P=0.002,0.006 and 0.004,respectively).The MAE was the smallest at 0.25 kHz,followed by 4 and 1 kHz,and the largest at 0°followed by 180°and±90°for both the elderly and the control group.The reaction time of the elderly group was longer than the control group,but there was no statistically significant difference.Conclusion The horizontal sound localization ability in the elderly with normal low and medium frequency hearing is lower than in normal hearing young and middle-aged people,with some variability associated with frequency and orientation.
