摘　　要：目的 探讨多层螺旋电子计算机断层扫描(MSCT)在原发性胆囊癌分期诊断及预后评估中的应用价值。方法 选取2018年1月-2021年1月收治的原发性胆囊癌患者68例作为研究对象,以手术病理学检查为“金标准”,于术前进行MSCT平扫和增强扫描,分析其在原发性胆囊癌分期诊断、预后评估中的应用价值。结果 MSCT对T分期的诊断结果:T_(1)期5例、T_(2)期9例、T_(3)期39例、T_(4)期15例,诊断的准确度为86.76%;MSCT对N分期的诊断结果:N_(0)期37例、N_(1)期17例、N_(2)期14例,诊断的准确度为89.71%;术后随访1年,死亡20例,死亡率为29.41%。MSCT对原发性胆囊癌患者术后1年生存率的AUC为0.873,灵敏度为60.00%,特异度为72.92%,准确率为69.12%。结论 MSCT对原发性胆囊癌术前分期诊断准确率高,且对预后有一定预测价值。Objective To explore the application value of multi-slice spiral computed tomography(MSCT)in the staging diagnosis and prognosis evaluation of primary gallbladder carcinoma.Methods A total of 68 patients with primary gallbladder carcinoma were enrolled as the research objects between January 2018 and January 2021.Taking surgical pathological examination as the golden standard,all underwent MSCT plain and enhanced scans before surgery.The application value in staging diagnosis and prognosis evaluation of primary gallbladder carcinoma was analyzed.Results The diagnostic results of MSCT on T stage:5 cases of T_(1) stage,9 cases of T_(2) stage,39 cases of T_(3) stage,and 15 cases of T_(4) stage,and the accuracy of diagnosis was 86.76%;the diagnostic results of MSCT on N stage:37 cases of N_(0) stage,17 cases of N_(1 )stage,and 14 cases of N_(2) stage,and the accuracy of diagnosis was 89.71%;20 cases died in the 1-year postoperative follow-up,and the mortality rate was 29.41%.The AUC of MSCT on the 1-year postoperative survival rate of primary gallbladder cancer patients was 0.873,with a sensitivity of 60.00%,specificity of 72.92%and accuracy of 69.12%.Conclusion MSCT has high diagnostic accuracy for preoperative staging of primary gallbladder carcinoma,which also has certain prognostic value.
