保乳术联合SLNB对45例早期乳腺癌患者的临床疗效

摘　　要：目的探究在早期乳腺癌患者中将保乳术与前哨淋巴结活检(sentinel lymph node,SLNB)进行联合应用的临床效果。方法随机选取2018年1月—2021年9月漳州市第二医院接收的早期乳腺癌患者为90例作为研究对象,将其分为两组,每组45例,给予传统乳腺根治术为对照组,给予保乳术联合SLNB治疗为研究组,对两组患者的各项手术指标、患者生活质量评分和并发症发生率进行比较分析。结果研究组患者的手术时间、手术出血量和拔管时间分别为(68.55±7.03)min、(85.45±9.55)mL和(5.45±0.56)d均优于对照组,差异有统计学意义(P<0.05)。研究组患者的社会功能、角色功能、健康功能、躯体功能、情感功能评分分别为(88.45±5.45)分、(86.56±5.44)分、(80.46±4.34)分、(90.45±4.33)分、(85.45±5.44)分,均比对照组评分高,差异有统计学意义(P<0.05)。同时,研究组中发生并发症的发生率为6.67%优于对照组,差异有统计学意义(χ2=5.414,P<0.05)。研究组患者的并发症发生率更低,治疗效果更好。结论在早期乳腺癌患者的治疗中将保乳术与SLNB进行联合应用,其效果显著,可促进患者恢复,提高患者生活质量,减少患者的并发症发生率,临床意义显著。Objective To investigate the clinical effect of combining breast-conserving surgery with SLNB(sentinel lymph node biopsy)in patients with early-stage breast cancer.Methods 90 cases of early breast cancer patients re⁃ceived by Zhangzhou Second Hospital from January 2018 to September 2021 were randomly selected as the research object,and they were divided into two groups,45 cases in each group.The traditional radical mastectomy was given as the control group,and breast conserving surgery combined with SLNB was given as the research group.The operation indicators,patient quality of life scores,and incidence of complications of the two groups were compared and analyzed.Results The surgical time,surgical bleeding volume,and extubation time of the study group patients were(68.55±7.03)min,(85.45±9.55)mL,and(5.45±0.56)d,respectively,which were superior to the control group,and the differ⁃ence was statistically significant(P<0.05).The social function,role function,health function,physical function,and emotional function scores of the study group patients were(88.45±5.45)points,(86.56±5.44)points,(80.46±4.34)points,(90.45±4.33)points,and(85.45±5.44)points,respectively,which were higher than the control group scores,and the difference was statistically significant(P<0.05).Meanwhile,the incidence of complications in the study group was 6.67%higher than that in the control group,and the difference was statistically significant(χ^(2)=5.414,P<0.05).The incidence of complications in the study group was lower and the treatment effect was better.Conclusion The com⁃bined application of breast-conserving surgery and SLNB in the treatment of early-stage breast cancer patients is effective,which can promote patients'recovery,improve their quality of life and reduce the incidence of complications,with significant clinical significance.
