小建中汤治疗虚劳腹痛机制探析

摘　　要：张仲景在《金匮要略》中论述了虚劳的证治,综合篇中关于病机和症状的论述,以及《素问》中所言“阳气者,烦劳则张”“劳则喘息汗出,外内皆越”,可以将虚劳的病机概括为精血内虚、阳气外浮。“清谷、亡血、失精”皆会导致精血内虚,而“脉大为劳”“脉浮大”“手足烦热”等症状皆表明了精血内虚而兼有阳气外浮。虚劳腹痛是虚劳病损在脾的表现,中焦血不足不能濡养、气不足不能温煦,如此则易产生里急、腹痛的症状,张仲景用甘温的小建中汤温养中焦之气血,同时方中的芍药也有益阴敛阳的作用,将外散于四肢的阳热之气敛降于中焦,使中焦气血充足。In Synopsis of Golden Chamber,ZHANG Zhongjing discussed the syndrome and treatment of consumptive disease.Combined with the analysis of the pathogenesis and symptoms of the disease in the article,and the words in Basic Conversation:“Overstrain will make yang qi hyperactive”,“Overstrain leads to panting,sweating and leakage of qi in the internal and external”,the pathogenesis of consumptive disease can be summarized as the internal deficiency of essence and blood and the floating of yang qi.“Undigested food,blood collapse and seminal emission”can lead to internal deficiency of essence and blood.The symptoms of“large pulse indicating consumptive disease”,“floating and large pulse”and“vexing heat of hands and feet”indicate internal deficiency of essence and blood with floating of yang qi.Abdominal pain is the manifestation of consumptive disease in spleen.Insufficient blood can not moisten,and insufficient qi can not warm,so it is easy to cause symptoms of acute abdominal pain.ZHANG Zhongjing used sweet and warm Xiaojianzhong Decoction(小建中汤)to warm and nourish qi and blood of middle energizer.At the same time,Shaoyao(Paeoniae Radix Alba)in the formula has the function of replenishing yin and astringing yang,astringing the qi of yang heat scattered in the extremities to the middle energizer,so that qi and blood in the middle energizer are sufficient.
