显微镜微血管减压术与神经内镜下微血管减压术治疗三叉神经痛的疗效比较

摘　　要：目的:本文旨在比较采用显微镜微血管减压术和神经内镜下微血管减压术治疗三叉神经痛的疗效差异。方法:在这项研究中,共收集了2019年6月-2022年12月邢台市人民医院接诊的40例三叉神经痛病人的资料,采用随机形式将其分成对照组和研究组,每组各有20例病人。对照组应用显微镜微血管减压术进行治疗,研究组应用神经内镜下微血管减压术进行治疗。统计分析两组治疗效果的差异性。结果:两组病人的治疗有效率相比存在显著差异(P<0.05);研究组中发生耳鸣1例,暂时性面部麻木1例,对照组中发生耳鸣3例,脑脊液漏2例,暂时性面部麻木4例,两组出现并发症例数相比存在显著差异(P<0.05)。结论:三叉神经痛是一种突发性、反复发作的疼痛疾病,主要表现为面部、嘴巴、牙齿、鼻子、眼睛等三叉神经支配区域的剧烈疼痛,常伴随着类似电击、刺痛等感觉,严重影响患者的生活质量,该疾病的病因可能涉及神经髓鞘和周围组织的压迫或刺激、炎症或损伤等因素,但具体原因尚未完全明确,采用神经内镜下微血管减压术治疗的效果明显优于显微镜微血管减压术治疗,且引发的并发症例数更少,因此其是一种安全且有效的治疗方法。Objective:To compare the efficacy of microvascular decompression under microscope and microvascular decompression under neuroendoscope in the treatment of trigeminal neuralgia.Methods:In this study,a total of 40 patients with trigeminal neuralgia admitted to Xingtai People’s Hospital from June 2019 to December 2022 were collected and randomly divided into control group and study group,with 20 patients in each group.The control group was treated with microvascular decompression and the study group was treated with microvascular decompression under neuroendoscope.The difference of treatment effect between the two groups was statistically analyzed.Results:There were significant differences in the effective rate between the two groups(P<0.05).In the study group,1 case of tinnitus and 1 case of temporary facial numbness occurred,while in the control group,3 cases of tinnitus,2 cases of cerebrospinal fluid leakage and 4 cases of temporary facial numbness occurred.There were significant differences in the number of complications between the two groups(P<0.05).Conclusion:Trigeminal neuralgia is a sudden and recurrent pain disease,mainly manifested as severe pain in the face,mouth,teeth,nose,eyes and other trigeminal innervation areas,often accompanied by similar electric shock,stinging and other feelings,seriously affecting the quality of life of patients,the cause of the disease may involve the compression or stimulation of the nerve myelin sheath and surrounding tissues,inflammation or damage and other factors.However,the specific reasons have not been fully clarified.The treatment effect of neuroendoscopic microvascular decompression is significantly better than that of microvascular decompression under microscope,and the number of complications is less,so it is a safe and effective treatment.
