摘　　要：目的探讨在鼻内镜辅助下行鼻泪道探通术治疗新生儿期泪囊炎的手术时机及临床疗效。方法选取大连市妇女儿童医疗中心(集团)收治的22例(43眼)新生儿期泪囊炎患儿,其中男6例、女16例,平均年龄22 d,所有患儿均有体温升高、患侧面部鼻部肿胀明显,患侧内眦以下泪囊区红肿及压痛,伴有大量脓性分泌物等症状,在保守治疗无效的情况下,行鼻内镜辅助下鼻泪道探通术,术后观察泪囊炎的治疗效果。结果22例泪囊炎患儿均行泪道探通术治疗,术后随访8个月~2年,所有患儿症状均好转。结论年龄<3个月患儿因泪道阻塞出现感染、红肿等症状时,保守治疗无效可行泪道探通,鼻内镜辅助手术可精确定位,避免鼻腔黏膜损伤。Objective To investigate the operative timing and clinical observations of nasal endoscopy-assisted probing of the nasolacrimal duct in the treatment of neonatal dacryocystitis.Methods 22 cases(43 eyes)of neonatal dacryocystitis in Dalian municipal women and children's medical center(group)were selected including 6 males and 16 females,with an average age of 22 days.All the children had symptoms such as elevated body temperature and obvious swelling of the nose on the affected side,redness,swelling and tenderness of the lacrimal sac area below the medial canthus of the ipsilateral side,accompanied by a large number of purulent secretions and other symptoms for which conservative treatment was ineffective.Under the circumstance of patients,nasal endoscopy-assisted inferior nasolacrimal duct probing was performed,and a therapeutic effect on dacryocystitis was observed after the operation.Results All the 22 children with dacryocystitis were treated with lacrimal duct probing.All the children's symptoms improved during the follow-up period of 8 months to 2 years.Conclusion In the case of infection,redness,and other symptoms due to lacrimal duct obstruction in children within 3 months,lacrimal duct probing is feasible if conservative treatment is ineffective,and an endoscopic nasal surgery can accurately locate and avoid nasal mucosal damage.
