血管角皮瘤101例激光治疗后临床疗效及不良反应分析

摘　　要：目的探讨血管角皮瘤患者激光治疗后的临床疗效及不良反应。方法收集第四军医大学西京皮肤医院激光美容中心2010年3月—2021年8月临床诊断为血管角皮瘤的患者101例,给予脉冲染料595 nm激光(PDL)、长脉冲1064 nm(Nd:YAG)以及脉冲CO_(2)激光单独或联合治疗,术前术后采集照片,对比分析治疗前后临床疗效及术后出现的不良反应。结果101例血管角皮瘤患者激光治疗后多数患者皮损明显改善或消退,痊愈率78.22%,有效率达到90.10%,16.83%的患者术后出现瘢痕、色素沉着斑或色素减退斑;以斑疹及丘疹、结节为主要皮损表现的患者有效率达98.36%及90.63%以上,不良反应率低,而以斑块为主的皮损出现不良反应发生率较高。结论激光治疗对血管角皮瘤患者治疗有效,可根据皮损类型选择治疗仪器,预判术后疗效。Objective The clinical efficacy and adverse reactions of 101 patients with angiokeratoma treated by laser were retrospectively analyzed.Methods A total of 101 patients with angiokeratoma from March 2010 to August 2021 at our department were treated by 595 nm pulsed dye laser(PDL)and long-pulse 1064 nm laser(Nd:YAG)and pulsed carbon dioxide laser(CO2)alone or in combination.The clinical efficacy and adverse reactions were observed and analyzed through the photos after therapy.Results Most of the 101 patients with angiokeratoma showed significant improvement or radically remission after laser treatment,the recovery rate was 78.22%,and the total effective rate was 90.10%.The scar,pigmentation and hypopigmentation were observed in 16.83%of patients.The effective rate of patients with macula,papules and nodules were 98.36%and 90.63%,and with rare adverse reaction,while much more adverse reactions were observed in the lesions dominated by plaque.Conclusion Laser therapy is effective in the treatment of angiokeratoma patients.Appropriated therapies were crucial for the patients with different type of lesions.The postoperative efficacy could be predicted by the morphological features of the lesion.
