摘　　要：目的:探索急性复发性胰腺炎(ARP)发病的危险因素,从而尽可能降低ARP发病率。方法:回顾性分析2017年1月—2018年1月广元市中心医院就诊的110例急性胰腺炎(AP)患者临床资料。依据发病次数分为初发组75例和复发组35例。收集其住院期间的临床资料进行统计学分析。结果:研究显示胆源性疾病、饮食不当、高C反应蛋白、高脂血症、长期饮酒是导致ARP的危险因素,差异有统计学意义(χ^(2)=5.188、5.059、4.365、4.716、4.190,P<0.05),其中胆源性疾病(OR=3.476,P<0.05)、高C反应蛋白水平(OR=3.217,P<0.05)、高脂血症(OR=2.890,P<0.05)是ARP的独立危险因素。结论:AP患者首次发病时患有胆囊或胆总管结石,胆管扩张,胆囊壁粗糙增厚,高胆红素,肝酶异常以及高C反应蛋白水平、高脂血症更容易再次复发。Objective:To explore the risk factors for the development of acute recurrent pancreatitis(ARP)so as to minimize the incidence of ARP.Methods:110 patients with acute pancreatitis(AP)seen in the hospital from January 2017 to January 2018 were retrospectively analyzed and divided into 75 cases primary group and 35 cases of recurrent group based on the number of epi⁃sodes.The clinical data during their hospitalization were collected for statistical analysis.Results:The study showed that cholestatic disease,improper diet,high CRP,hyperlipidemia,and chronic alcohol consumption were risk factors for ARP,with statistically sig⁃nificant difference(χ^(2)=5.188,5.059,4.365,4.716,4.190,P<0.05).Among them,cholestatic disease(OR=3.476,P<0.05),high C-reactive protein level(OR=3.217,P<0.05),and hyperlipidemia(OR=2.890,P<0.05)were independent risk factors for ARP.Conclusion:AP patients with gallbladder or common bile duct stones,dilated bile ducts,rough and thickened gallbladder walls,high bilirubin,abnormal liver enzymes and high C-reactive protein levels,and hyperlipidemia at the time of first presentation are more likely to relapse.
