D2根治术联合胃裸区清扫治疗M0近端胃癌的临床效果及对血清肿瘤标志物的影响

摘　　要：目的:分析D2根治术辅助胃裸区清扫方案用于MO近端胃癌的疗效。方法:选取2018年3月—2021年5月佳木斯市中心医院收治的80例M0近端胃癌患者作为研究对象,依照随机数表法分为两组,对照组(D2根治术)40例、观察组(联合开展胃裸区清扫治疗)40例,对比不同方案对疗效及血清肿瘤标志物水平变化的影响。结果:观察组有效率高于对照组,差异有统计学意义(χ^(2)=4.114,P<0.05);两组患者生存率比较,观察组更高,差异有统计学意义(χ^(2)=4.528,P<0.05);治疗后,观察组血清肿瘤标志物指标较低,差异有统计学意义(t=10.076、12.184、5.716,P<0.05);治疗后,观察组生活质量评分较高,差异有统计学意义(t=2.798、3.092、3.150、3.331、3.073、3.888、2.892、3.899,P<0.05);治疗后,观察组睡眠质量评分较低,差异有统计学意义(t=3.192、3.665、2.476、2.501、2.214、3.315、4.345,P<0.05)。结论:D2根治术联合胃裸区清扫治疗在M0近端胃癌治疗中,可加快疾病恢复速度,提高患者接受程度。Objective:To analyze the efficacy of D2 radical surgery adjuvant to gastric bare zone debridement protocol for proximal gastric cancer in MO.Methods:80 patients with M0 proximal gastric cancer admitted to the hospital from March 2018 to May 2021 were selected as the study subjects and divided into 40 cases in the control group(D2 radical surgery)and 40 cases in the observation group(combined with gastric naked zone debridement treatment)according to the random number table method.The ef⁃fects of different regimens on efficacy and changes in serum tumor marker levels were compared.Results:The efficiency rate of the observation group was higher than that of the control group,and the difference was statistically significant(χ^(2)=4.114,P<0.05).The survival rate was higher in the observation group,and the difference was statistically significant(χ^(2)=4.528,P<0.05).After treatment,serum tumor marker indexes were lower in the observation group,and the difference was statistically significant(t=10.076,12.184,5.716,P<0.05).After treatment,the quality of life scores were higher in the observation group,with statistically significant differences(t=2.798,3.092,3.150,3.331,3.073,3.888,2.892,3.899,P<0.05).After treatment,the observation group had lower sleep quality scores,with statistically significant differences(t=3.192,3.665,2.476,2.501,2.214,3.315,4.345,P<0.05).Conclusion:D2 radical surgery combined with gastric bare zone debridement therapy in the treatment of M0 proximal gas⁃tric cancer to accelerate disease recovery and improve patient acceptance.
