摘　　要：肝癌治疗原则主要依据临床分期来选择不同的治疗方案,早中期患者以手术及局部治疗为主,中晚期患者以综合性治疗为主。已知靶向治疗与免疫治疗可以提高晚期肝癌患者的生存期并改善预后。在分子靶向治疗及免疫治疗时代,识别预测性生物标志物对于成功实施个性化医疗至关重要。而对于肝癌治疗的多元化,如何对肝癌治疗反应、预后进行有效预测,是亟待解决的临床问题。影像组学以量化的方式揭示肉眼无法识别的与微观组织异质性间接相关的宏观组织异质性,能够对肿瘤的生物学特征进行有效评估,以用于肿瘤分级分期、疗效评估和预后预测等。文章就影像组学与肝癌病理特征、基因表型及免疫特征、治疗反应及预后等问题的相关性研究进行综述。The treatment principle of liver cancer is mainly based on clinical stage.The early and middle stage patients were mainly treated by surgery and local treatment.The middle and late stage patients were mainly treated by comprehensive treatment.It is known that the survival and prognosis of patients with advanced hepatocellular carcinoma can be improved by targeted therapy and immunotherapy.In the era of molecular targeted therapy and immunotherapy,identifying predictive biomarkers are important for the successful implementation of personalized medicine.For the diversification treatment of hepatocellular carcinoma,how to effectively predict the treatment response and prognosis of hepatocellular carcinoma is an urgent clinical problem.Radiomics reveals in a quantitative manner the macroscopic tissue heterogeneity that is indirectly related to microscopic tissue heterogeneity,which cannot be recognized by the naked eye.The classification and stage of the tumors can be evaluated effectively,as well as the prognosis of the patients.This article reviews the correlation between radiomics and pathological characteristics,genotype and immune characteristics,treatment response and prognosis of hepatocellular carcinoma.
