胃窦浆肌瓣转移幽门成形术对近端胃切除术后的干预效果

摘　　要：目的 分析胃窦浆肌瓣转移幽门成形术对胃癌患者近端胃切除术后的恢复情况干预效果。方法 选择2019年1月至2021年12月之间在睢县中医院接受近端胃切除手术治疗的胃癌患者128人。患者依据机数字表法实施分组。对照组患者(64人)采用常规的近端胃切除手术方法。观察组患者(64人)近端胃切除术后再联合使用胃窦浆肌瓣转移幽门成形术。两组患者各类术后恢复指标的对比:胃管留置时间,胃管引流量,肠道功能恢复时间,胃瘫发生情况,术后住院时间长度。术后3个月,两组患者胃排空障碍、返流性食管炎发生情况的对比。分析不同手术方法患者的治疗满意度差异。结果 观察组在胃管留置时间、胃管引流量、肠道功能恢复时间、术后住院时间方面的数据均低于对照组,差异有统计学意义(P <0.05)。两组胃瘫发生率的数据差异无统计学意义(P> 0.05)。手术后3个月,观察组的胃排空障碍、返流性食管炎发生率均低于对照组,差异有统计学意义(P <0.05)。观察组患者对于治疗的满意度评分显著高于对照组患者,差异有统计学意义(P <0.05)。结论 胃窦浆肌瓣转移幽门成形术对于接受近端胃切除术的胃癌患者具有良好的效果,能够有效的促进术后消化道功能的恢复,降低术后并发症的危险,提升接受治疗患者的满意程度。Objective To analyze the effect of pylorplasty with gastric antrum seromuscular flap transfer on the recovery of patients with gastric cancer after proximal gastrectomy.Methods A total of 128 patients with gastric cancer who underwent proximal gastrectomy in Suixian Chinese Medicine Hospital between January 2019 and December 2021 were selected.The patients were divided into groups according to the random number table method.Control group(64 patients)received conventional proximal gastrectomy.Observation group(64 patients)underwent proximal gastrectomy and pyloroplasty combined with antrum pulp muscle flap transfer.Various postoperative recovery indexes were compared between the two groups:indwelling time of gastric tube,gastric tube drainage volume,recovery time of intestinal function,occurrence of gastroparesis,and length of postoperative hospital stay.Three months after surgery,the gastric emptying disorder and reflux esophagitis were compared between the two groups.To analyze the difference of treatment satisfaction of patients with different surgical methods.Results The results of the observation group were lower than those of the control group in terms of indentation time of gastric tube,drainage volume of gastric tube,recovery time of intestinal function and postoperative hospital stay,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of gastroparesis between the two groups(P>0.05).Three months after the operation,the incidence of gastric empelation disorder and reflux esophagitis in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The satisfaction score of observation group was significantly higher than that of control group,the difference was statistically significant(P<0.05).Conclusion Gastric antrum muscle flap transfer pylorplasty has good effect on gastric cancer patients receiving proximal gastrectomy,which can effectively promote the recovery of digestive tract function after su
