胃镜下活检在胃癌患者诊断中的应用价值

摘　　要：目的探讨胃镜下活检在胃癌患者诊断中的应用价值。方法选取2019年1月至2022年1月我院收治的86例疑似胃癌患者,均行胃镜下活检及手术病理检查。以手术病理检查结果作为金标准,统计胃镜下活检胃癌检出情况及诊断效能。采用κ检验分析胃镜下活检诊断胃癌与手术病理检查结果的一致性。结果86例疑似胃癌患者经手术病理检查确诊72例,经胃镜下活检确诊71例,两种检查方法胃癌检出率比较差异无统计学意义(P>0.05)。胃镜下活检诊断胃癌的灵敏度为98.61%(71/72),特异度为100.00%(14/14),准确度为98.84%(85/86),阳性预测值为100.00%(71/71),阴性预测值为93.33%(14/15);κ检验显示,胃镜下活检诊断胃癌与手术病理检查一致性极好(κ=0.959,P=0.000)。结论胃镜下活检诊断胃癌的灵敏度、特异度及准确度均较高,且诊断结果与手术病理检查一致性极好,可作为胃癌诊断的重要手段。Objective To explore the application value of gastroscopic biopsy in the diagnosis of patients with gastric cancer.Methods 86 patients with suspected gastric cancer admitted to our hospital from January 2019 to January 2022 were selected,and given gastroscopic biopsy and surgical pathological examination.Taking the surgical pathological examination results as the gold standard,the detection and diagnostic efficacy of gastroscopic biopsy for gastric cancer were counted.k test was used to analyze the consistency between gastroscopic biopsy and surgical pathological examination in the diagnosis of gastric cancer.Results Among 86 patients with suspected gastric cancer,72 cases were confirmed by surgical pathological examination and 71 cases were confirmed by gastroscopic biopsy.No statistical difference was found in the detection rate of gastric cancer between the two examination methods(P>0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of gastroscopic biopsy in the diagnosis of gastric cancer were 98.61%(71/72),100.00%(14/14),98.84%(85/86),100.00%(71/71)and 93.33%(14/15),respectively.k test showed that gastroscopic biopsy in the diagnosis of gastric cancer has excellent consistency with surgical pathological examination(k=0.959,P=0.000).Conclusions Gastroscopic biopsy in the diagnosis of gastric cancer has higher sensitivity,specificity and accuracy,which has excellent consistency with surgical pathological examination,and can be used as an important method for the diagnosis of gastric cancer.
