摘  要:目的:探讨二维斑点追踪成像技术在评估射血分数保留心力衰竭(HFpEF)模型小鼠心肌功能中的应用价值。方法:16只C57BL/6小鼠被随机分为HFpEF模型组(8只)和正常对照组(8只),HFpEF模型组小鼠予以皮下注射醋酸去氧皮质酮(DOCA)及高盐高脂饮食喂养,正常对照组予以普通饲料及饮用水喂养,造模12周后行超声心动图、心衰标志物、血压及心脏称重等检查。通过高频超声测定左室舒张末内径(LVIDd)、左房(LA)、室间隔(IVS)、左室后壁(LVPW)、左室射血分数(LVEF)、缩短率(FS)、舒张期二尖瓣E峰和A峰、组织多普勒二尖瓣环E’峰、左房容积指数(LAVI)和E/E’比值,应用EchoPAC工作站分析左室乳头肌水平短轴切面各节段的收缩期峰值径向应变(PRS)及径向应变率(PRSR),利用动物血压计测量小鼠收缩压(SBP)和舒张压(DBP),利用全自动生化分析仪检测小鼠血清脑钠肽(BNP)水平,最后处死小鼠称取心脏质量。结果:与正常对照组相比,HFpEF模型组小鼠的心脏质量、SBP、DBP及血清BNP均明显升高,LVIDd扩大、IVS及LVPW增厚、E/E’比值显著升高,左室乳头肌短轴切面各节段心肌PRS及PRSR测值均明显降低。结论:二维斑点追踪成像技术可以准确量化及评估HFpEF模型小鼠的心肌功能。Objective:To investigate the value of two-dimensional speckle tracking imaging in assessing myocardial function in heart failure with preserved ejection fraction(HFpEF)model mice.Methods:Sixteen C57BL/6 mice were randomly divided into HFpEF group(n=8)and control group(n=8).The HFpEF model group was subcutaneously injected with deoxycorticosterone acetate(DOCA)and fed with high salt and high fat diet,while the control group was fed conventionally.Echocardiography and heart failure indexes were measured after 12 weeks.Left ventricular internal diastolic diameter(LVIDd),left atrium(LA),interventricular septum(IVS),left ventricular posterior wall(LVPW),left ventricular ejection fraction(LVEF),fractional shortening(FS),diastolic mitral spectrum E and A peaks,tissue Doppler mitral annulus E'peaks,left atrial volume index(LAVI)and E/E'ratio were measured with conventional echocardiography.Peak radical strain(PRS)and peak radical strain rate(PRSR)of each segment were analyzed using EchoPAC workstation in the left ventricular short axis views at the papillary muscle level which were measured at systolic period.The serum brain natriuretic peptide(BNP)levels of mice were measured by automatic biochemical analyzer.Systolic blood pressure(SBP)and diastolic blood pressure(DBP)were measured using an animal sphygmomanometer.In the final step,mice were executed to obtain heart weights.Results:Compared with the normal control group,SBP,DBP,serum BNP,heart weight,and E/E'ratio were significantly increased,LVIDd were significantly enlarged,IVS and LVPW were significantly thickened,while PRS and PRSR measurements were significantly decreased in all segments of the left ventricular papillary muscle in the short-axis views.Conclusion:Two-dimensional speckle tracking imaging can accurately quantify myocardial function in the HFpEF model mice.
