摘  要:目的:探索影响食管鳞状细胞癌(ESCC)患者预后的代谢相关基因,构建ESCC患者的预后风险模型。方法:从癌症基因组图谱(TCGA)数据库下载食管癌的转录组数据和临床病例资料。从转录组数据中提取出代谢相关基因的数据,筛选ESCC组织和正常组织中差异表达的代谢基因。运用单因素COX分析、Lasso回归分析、多因素COX分析构建预后风险模型,并用受试者工作特征(ROC)曲线评估代谢相关基因预后模型的预测能力,并对高低风险组进行GSEA分析。结果:肿瘤组和对照组共存在280个差异表达的代谢相关基因,其中乙醛脱氢酶1A1(ALDH1A1)、黑酸1,2-二加氧酶(HGD)以及DNA引物酶多肽1(PRIM1)与ESCC的预后相关。Kaplan-Meier分析显示,相较于低风险组,高风险组的总体生存期降低(P<0.01)。ROC曲线分析及多因素COX回归分析表明,该预后模型具有良好的预测效能[曲线下面积(AUC)=0.741],可以作为一个独立的预后因素(HR=11.66,P<0.001)。结论:基于代谢相关基因构建的预后风险模型可以作为ESCC患者的预后因素,为寻找新的食管癌治疗靶点提供了方向。Objective:To explore the metabolic-related genes that affect the prognosis of patients with esophageal squamous cell carcinoma(ESCC)and construct a prognostic risk model for patients with esophageal squamous cell carcinoma.Methods:The transcriptome data and clinical case data of esophageal cancer were downloaded from the Cancer Genome Atlas(TCGA)database.The data of metabolism-related genes were extracted from the transcriptome data,and the metabolic genes that were differentially expressed in ESCC and normal tissues were screened out.Single-factor COX analysis,Lasso regression analysis,and multi-factor COX analysis were used to construct a prognostic risk model,and receiver operating characteristic(ROC)curve were used to evaluate the predictive ability of metabolic-related gene prognostic models.GSEA analysis was performed on the high and low risk groups.Results:There are 280 differentially expressed metabolic-related genes in the tumor group and the control group.Among them,Alaldehyde dehydrogenase 1 A1(ALDH1A1),homogentisate 1,2-dioxygenase(HGD),and DNA primase polypeptide 1(PRIM1)were related to the prognosis of ESCC.Kaplan-Meier analysis showed that compared with that in the low-risk group,the overall survival of the high-risk group was reduced(P<0.01).ROC curve analysis and multivariate COX regression analysis showed that the prognostic model had good predictive power(AUC=0.741)and could be used as an independent prognostic factor(HR=11.662,P<0.05).Conclusion:The prognostic risk model constructed based on metabolism-related genes can be used as a prognostic factor for patients with ESCC,providing a direction for finding new therapeutic targets for esophageal cancer.
