摘 要:目的探讨CT与磁共振成像(MRI)诊断骨肉瘤的效果及其评估浸润侵犯范围的价值。方法抽取郑州市骨科医院2015年1月至2020年12月收治的34例经手术病理证实的骨肉瘤患者。所有患者术前均接受CT与MRI检查,比较二者的检查结果。结果34例骨肉瘤患者中,MRI与CT诊断骨肉瘤的符合率比较差异未见统计学意义(P>0.05);两种方式骨肉瘤征象(骨质破坏、骨膜反应、骨膜三角、软组织肿块)检出率比较,差异未见统计学意义(P>0.05),但CT肿瘤骨检出率(100%,34/34)高于MRI(0,0/34),P<0.05;两种方法在骨肉瘤浸润侵犯范围(骨皮质、外骨膜、骨骼肌间筋膜)的诊断结果中比较,差异未见统计学意义(P>0.05),但MRI在骨骼肌的检出率(70.59%,24/34)高于CT(47.06%,16/34),P<0.05。结论骨肉瘤采取CT与MRI诊断均有较好的价值,诊断符合率都较高,但CT可更好地检出骨肉瘤征象情况,MRI则可更好地检出浸润侵犯范围,临床上可结合二者进行联合诊断。Objective To investigate the diagnostic effect of CT and magnetic resonance imaging(MRI)for osteosarcoma,and the value in the evaluation of the extent of invasion.Methods Thirty-four patients with osteosarcoma confirmed by surgery and pathology in Zhengzhou Orthopedics Hospital from January 2015 to December 2020 were selected.All patients received CT and MRI examination before operation,and the results of the two methods were compared.Results Among the 34 patients with osteosarcoma,there was no significant difference in coincidence rate between MRI and CT in the diagnosis of osteosarcoma(P>0.05).There was no significant difference in the detection rate of osteosarcoma signs(bone destruction,periosteal reaction,periosteal triangle,soft tissue mass)between the two methods(P>0.05),but the detection rate of tumor bone by CT(100%,34/34)was higher than that by MRI(0,0/34),P<0.05.There was no statistically significant difference in the diagnosis of osteosarcoma invasion range(bone cortex,periosteum,skeletal muscle fascia)between the two methods(P>0.05),but the detection rate of skeletal muscle by MRI(70.59%,24/34)was higher than that by CT(47.06%,16/34),P<0.05.Conclusions Both CT and MRI are valuable in the diagnosis of osteosarcoma,and the diagnostic coincidence rate of the two methods are high.However,CT can better detect the signs of osteosarcoma,while MRI can better detect the range of invasion.The clinical diagnosis can be performed based on the combination of the two methods.
