摘 要:目的:探究人羊膜上皮干细胞(hAECs)联合富血小板血浆(PRP)对宫腔粘连大鼠的治疗效果,为临床中优化宫腔粘连治疗方法理论基础。方法:选用SPF级雌性SD大鼠共40只,依照随机数字表法随机分为空白组(A组)、假手术组(B组)、hAECs组(C组)、PRP组(D组)、hAECs+PRP组(E组),每组各8只。A组不做任何处理,B组仅做麻醉、开关腹腔处理,C组、D组、E组使用搔刮法制备宫腔粘连模型。随后C组进行hAECs治疗,D组注射等量生理盐水。D组进行PRP治疗,C组注射等量生理盐水。E组进行hAECs联合PRP治疗。观察比较两组大鼠子宫内膜组织基质金属蛋白酶-9(MMP-9)、盘状结构域受体2(DDR2)蛋白表达水平、PI3K/Akt/mTOR通路相关mRNA表达量、血清白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平以及各组大鼠妊娠孕囊数量比较。结果:与A组相比,B组的子宫内膜组织MMP-9、DDR2蛋白表达水平、p-mTOR mRNA、p-Akt mRNA水平以及血清IL-6、IL-8水平比较无差异(P>0.05);C组、D组、E组大鼠的子宫内膜组织MMP-9、DDR2蛋白表达水平明显下降,IL-6、IL-8水平、p-mTOR mRNA、p-Akt mRNA表达升高(P<0.05)。其中,E组大鼠MMP-9、DDR2蛋白表达水平明显高于C组、D组,IL-6、IL-8水平及p-mTOR mRNA、p-Akt mRNA表达低于C组、D组(P<0.05)。C组、D组大鼠MMP-9、DDR2蛋白表达水平、血清IL-6、IL-8水平及p-mTOR mRNA、p-Akt mRNA表达比较(P>0.05)。妊娠孕囊数量方面,与A组比较,B组大鼠妊娠孕囊数量。结论:hAECs联合PRP治疗可能通过上调宫腔粘连大鼠子宫内膜组织MMP-9、DDR2蛋白表达水平并下调p-mTOR mRNA、p-Akt mRNA表达,同时降低炎性因子IL-6、IL-8水平,进而提高妊娠孕囊数量,起到治疗作用。Objective:To investigate the therapeutic effect of human Amniotic Epithelial cells(hAECs)combined with platelet rich Plasma(PRP)on intrauterine adhesion in rats,so as to provide a theoretical basis for optimizing the treatment of intrauterine adhesion in clinical treatment.Methods:40 SPF female SD rats were randomly divided into blank group(group A),sham operation group(group B),hAECs group(group C),PRP group(group D)and hAECs+PRP group(group E)according to the random number table method,with 8 rats in each group.Group A did not do any operation,group B did anesthesia and open surgery,groups C,D,E used scratching method to make intrauterine adhesion model.Subsequently,groups C was treated with hAECs,and group D was injected with the same volume of normal saline.Groups D was treated with PRP,and group C was injected with the same volume of normal saline.Group E was treated with hAECs combined with PRP.The protein expression levels of MMP-9 and DDR2,related mRNA expression levels of PI3K/Akt/mTOR pathway,serum levels of IL-6 and IL-8,and the number of gestational sacs in each group were observed and compared between the two groups.Results:Compared with group A,there were no differences in the protein expression levels of MMP-9 and DDR2,p-MTOR mRNA and p-Akt mRNA levels,and serum IL-6 and IL-8 levels in group B(P>0.05).The protein expression levels of MMP-9 and DDR2 in endometrial tissues of rats in groups C,D and E were decreased,while the levels of IL-6 and IL-8,p-mTOR mRNA and p-Akt mRNA were increased(P<0.05).The protein expression levels of MMP-9 and DDR2 in group E were significantly higher than those in groups C and D,and the levels of IL-6 and IL-8,p-mTOR mRNA and p-Akt mRNA were lower than those in groups C and D(P<0.05).There were no differences in the protein expression levels of MMP-9 and DDR2,serum IL-6 and IL-8 levels,and the mRNA expression of p-mTOR and p-Akt between groups C and D(P>0.05).In terms of the number of gestational sacs,compared with group A,the number of gestational sac of rats in group
