中药熏洗辅助康复训练对痉挛型脑性瘫痪患儿下肢功能的影响

摘　　要：目的:探讨痉挛型脑性瘫痪患儿应用中药熏洗配合康复训练,对其下肢运动功能、下肢肌张力及表面肌电的影响。方法:选取2021年1月至2022年5月期间本院83例痉挛型脑性瘫痪患儿作为研究对象,以抽签的方法将其分组。对照组41例给予常规康复训练,研究组42例组联合中药熏洗辅助治疗。8 w后,对比两组患儿下肢运动功能、下肢肌张力、下肢表面肌电。结果:干预后,两组患儿粗大运动功能测试中D区、E区分值均升高,但研究组升高幅度高于对照组(P<0.05);干预后,两组患儿内收肌、腓肠肌、腘绳肌分值均降低,但观察组降低幅度高于对照组(P<0.05);干预后,两组患儿主动状态下腓肠肌积分肌电值(Integrated electromyogram,iEMG)、胫骨前肌iEMG均升高,被动状态下腓肠肌iEMG、胫骨前肌iEMG均降低,但观察组升高或降低幅度高于对照组(P<0.05)。结论:中药熏洗辅助康复训练可有效提高痉挛型脑性瘫痪患儿下肢运动功能,降低肌张力,增强下肢肌力,值得临床推广并应用。Objective:To investigate the effects of fumigation with traditional Chinese medicine combined with rehabilitation training on lower extremity motor function,lower extremity muscle tone and surface electromyography in children with spastic cerebral palsy.Methods:A total of 83 children with spastic cerebral palsy from January 2021 to May 2022 were selected as research objects and divided into groups by lottery.The control group(41 cases)received routine rehabilitation training,and the study group(42 cases)received combined fumigation with traditional Chinese medicine.After 8 weeks,lower extremity motor function,lower extremity muscle tone and lower extremity surface electromyography were compared between the two groups.Results:After intervention,the difference values of area D and E in the gross motor function test of the two groups were increased,but the increase rate of the study group was higher than that of the control group(P<0.05).After intervention,the scores of adductor muscle,gastrocnemius muscle and hamstring muscle in the two groups were decreased,but the reduction in the study group was higher than that in the control group(P<0.05).After intervention,the Integrated electromyogram(iEMG)of gastrocnemius muscle and tibialis anterior muscle in the active state were increased,and the iEMG of gastrocnemius muscle and tibialis anterior muscle in the passive state were decreased,but the increase or decrease in the study group was higher than that in the control group(P<0.05).Conclusion:Traditional Chinese medicine fumigation and washing assisted rehabilitation training can effectively improve lower limb motor function,reduce muscle tension and enhance lower limb muscle strength in children with spastic cerebral palsy,which is worthy of clinical promotion and application.
