全瓷冠修复与金属烤瓷冠修复在牙隐裂伴牙髓炎患者根管术后的应用效果  

Effects of all-ceramic crown restoration and metal-boned ceramic crown restoration after root canal surgery on patients with cracked teeth complicated by pulpitis

摘　　要：目的探讨并比较全瓷冠修复与金属烤瓷冠修复在牙隐裂伴牙髓炎根管术后的应用效果。方法抽取2019年6月至2021年6月常熟市第一人民医院收治的牙隐裂伴牙髓炎患者82例,按随机数字表法分为对照组与观察组,每组41例。两组患者均先行根管治疗,对照组术后行金属烤瓷冠修复,观察组术后行全瓷冠修复。随访6个月,比较两组修复效果、牙周指标及咀嚼功能。结果观察组总有效率(95.12%,39/41)高于对照组(80.49%,33/41),P<0.05;修复后6个月,观察组菌斑指数[(0.85±0.12)分]、出血指数[(0.71±0.11)分]、牙周探诊深度[(1.05±0.14)mm]及牙龈指数[(0.82±0.12)分]均低于对照组[(1.14±0.21)分、(0.93±0.13)分、(1.32±0.21)mm、(1.08±0.19)分],P<0.05;观察组修复后咬合力[(60.42±5.17)kg]、咀嚼效率[(90.07±6.38)%]均高于对照组[(53.54±5.09)kg、(82.22±6.12)%],差异有统计学意义(P<0.05)。结论全瓷冠修复在牙隐裂伴牙髓炎根管术后患者中的应用效果更佳,能够减轻牙周组织损伤,促进咀嚼功能恢复。Objective To investigate and compare the effect of all-ceramic crown restoration and metal-boned ceramic crown restoration after root canal surgery for cracked teeth with pulpitis.Methods A total of 82 patients with cracked teeth complicated by pulpitis admitted to Changshu First People’s Hospital from June 2019 to June 2021 were selected,and they were divided into control group and observation group according to the random number table method,with 41 patients in each group.Both groups were treated by root canal surgery firstly.The control group was repaired with metal boned ceramic crowns,and the observation group was repaired with all-ceramic crowns.All the patients were followed up for 6 months,while the restoration effect,periodontal indicators,masticatory function were compared between the two groups.Results The total effective rate of the observation group(95.12%,39/41)was higher than that of the control group(80.49%,33/41),P<0.05.The plaque index,bleeding index,periodontal probing depth and gingival index of the observation group were 0.85±0.12,0.71±0.11,(1.05±0.14)mm and 0.82±0.12,respectively,all of which were lower than the 1.14±0.21,0.93±0.13,(1.32±0.21)mm and 1.08±0.19 of the control group 6 months after repair(P<0.05).In the observation group,the occlusal force was(60.42±5.17)kg,and masticatory efficiency was(90.07±6.38)%after repair,and the two indicators were higher than the(53.54±5.09)kg and(82.22±6.12)%in the control group,with statistical differences(P<0.05).Conclusions All-ceramic crown restoration is more effective after root canal surgery for cracked teeth with pulpitis,which can reduce periodontal tissue damage and promote masticatory function recovery.
