腹腔镜卵巢囊肿剥除术治疗卵巢良性肿瘤的临床价值

摘　　要：目的分析卵巢良性肿瘤应用腹腔镜卵巢囊肿剥除术治疗的临床价值。方法随机选取2016年1月—2022年1月在徐州市妇幼保健院治疗的卵巢良性肿瘤患者80例进行研究,其中对照组40例采用开腹卵巢囊肿剥除术,观察组40例采用腹腔镜卵巢囊肿剥除术,对两组患者围术期相关指标、术后疼痛情况及对卵巢功能的影响予以观察比较。结果观察组手术时间(63.41±9.36)min、排气时间(18.35±3.41)h、下床时间(35.89±3.29)h、住院时间(5.31±1.28)d均短于对照组,术中出血量(67.12±12.16)mL低于对照组,差异有统计学意义(t=4.041、4.529、4.254、4.520、4.705,P<0.05)。观察组术后1、3 d疼痛评分均低于对照组,差异有统计学意义(P<0.05)。观察组术后3个月雌二醇(E2)高于对照组,促黄体生成素(LH)、卵泡刺激素(FSH)水平均低于对照组,差异有统计学意义(P<0.05)。结论卵巢良性肿瘤行卵巢囊肿剥除术时,腹腔镜手术与开腹手术相比,具有手术时间短、术中出血少、手术创伤小、术后疼痛轻、恢复快、治疗时间短等优势,且腹腔镜手术不会增加对患者卵巢功能的影响。Objective To analyze the clinical value of applying laparoscopic ovarian cyst debridement for the treatment of benign ovarian tumors.Methods 80 cases of benign ovarian tumor patients treated in Xuzhou Maternal and Child Health Hospital from January 2016 to January 2022 were randomly selected for the study.Among them,the control group of 40 cases underwent open ovarian cyst removal surgery,while the observation group of 40 cases underwent laparoscopic ovarian cyst removal surgery.The two groups of patients perioperative related indicators,postoperative pain,and the effect on ovarian function were observed and compared.Results The operative time(63.41±9.36)min,time to exhaustion(18.35±3.41)h,time to bed(35.89±3.29)h,and hospital stay(5.31±1.28)d were shorter in the observation group than in the control group,and the intraoperative bleeding(67.12±12.16)mL was lower than that in the control group,and the difference was statistically significant(t=4.041,4.529,4.254,4.520,4.705,P<0.05).The Postoperative pain scores at 1 d and 3 d after surgery were lower in the observation group than in the control group,and the difference was statistically significant(P<0.05).Estradiol(E2)was higher,and luteinizing hormone(LH)and follicle-stimulating hormone(FSH)levels were lower in the observation group than in the control group 3 months after surgery,and the difference was statistically significant(P<0.05).Conclusion When ovarian cyst debridement is performed for benign ovarian tumors,laparoscopic surgery has the advantages of shorter operative time,less intraoperative bleeding,less surgical trauma,less postoperative pain,faster recovery and shorter treatment time compared with open surgery.And laparoscopic surgery will not increase the impact on ovarian function of patients.
