养正合剂辅助CAG化疗方案治疗老年急性髓系白血病的效果

摘　　要：目的 探讨养正合剂辅助CAG化疗方案治疗老年急性髓系白血病(AML)的效果及对癌因性疲乏、免疫功能的影响。方法 选取2021年1月~2022年1月吉林大学第二医院肿瘤血液科治疗的82例老年AML患者作为研究对象,使用随机数字表法把患者分为对照组和观察组,各41例。对照组接受CAG化疗,观察组采用养正合剂辅助CAG化疗,均治疗2个疗程。比较两组治疗后的临床疗效、不良反应发生率和治疗前后的癌因性疲乏程度;检测两组患者的CD3^(+)、CD4^(+)、CD8^(+)、免疫球蛋白(Ig)A、IgG、IgM水平。结果 观察组治疗总有效率高于对照组(P<0.05);治疗后,观察组的情绪疲乏、认知疲乏、感觉疲乏及行为疲乏评分低于治疗前且低于对照组(P<0.05),CD3^(+)、CD4^(+)水平高于对照组(P<0.05),CD8^(+)水平低于对照组(P<0.05),IgA、IgG、IgM水平高于对照组(P<0.05),血小板计数高于对照组(P<0.05);治疗期间,观察组骨髓抑制等不良反应发生率均低于对照组(P<0.05)。结论 养正合剂辅助CAG化疗方案可以明显提高老年AML患者的临床疗效,改善癌因性疲乏和免疫功能。Objective To explore the effect of CAG chemotherapy combined with Yangzheng Mixture on older patients with acute myeloid leukemia(AML)and its influences on cancer-related fatigue and immune function.Methods A total of 82 older patients with AML treated in the Second Hospital of Jilin University were enrolled between January 2021 and January 2022.According to random number table,the patients were divided into a control group(41 cases)and an observation group(41 cases).The control group received CAG chemotherapy,and the observation group received CAG chemotherapy plus Yangzheng Mixture.All patients were treated for 2 courses.The clinical efficacy,incidence of adverse reactions,and the degree of cancer-related fatigue before and after treatment were compared between the two groups.The levels of CD3^(+),CD4^(+),CD8^(+),Ig A,IgG and IgM in the two groups were measured.Results The overall response rate in the observation group was higher than that in the control group(P<0.05).After treatment,the scores of emotional fatigue,cognitive fatigue,sensory fatigue and behavioral fatigue in the observation group were lower than those before treatment and lower than those in the control group(P<0.05).Compared with the control group,the observation group had higher levels of CD3^(+),CD4^(+)(P<0.05),lower level of CD8^(+)(P<0.05),higher levels of IgA,IgG and IgM(P<0.05),higher PLT count(P<0.05),and lower incidence of adverse reactions(P<0.05).Conclusion CAG chemotherapy combined with Yangzheng Mixture may significantly improve the clinical efficacy,cancer-related fatigue and immune function in the treatment of AML.
