食管癌患者MMPs/TIMPs表达水平及其与Hp感染的关联

摘　　要：目的探讨食管癌患者基质金属蛋白酶(MMPs)与组织基质金属蛋白酶的抑制剂(TIMPs)表达水平及其与幽门螺杆菌(Hp)感染的关联,为食管癌的病因学研究和靶向治疗提供更多科学依据。方法选取2020年6月-2022年5月于遂宁市中心医院就诊的食管癌患者50例和食管炎患者50例分别为食管癌组和食管炎组,另选择50名健康体检者为对照组,检测Hp感染情况,实时荧光定量聚合酶链式反应(PCR)检测MMPs/TIMPs mRNA相对表达量和蛋白相对表达水平。结果对照组、食管炎组、食管癌组Hp阳性率分别为14.0%、36.0%、84.0%;与对照组比较,食管炎组、食管癌组MMP-2、MMP-9、TIMP-1、TIMP-2 mRNA相对表达量与蛋白相对表达水平均升高(P<0.05);与食管炎组比较,食管癌组MMP-2、MMP-9、TIMP-1、TIMP-2 mRNA相对表达量与蛋白相对表达水平均升高(P<0.05);Hp阳性食管癌患者MMP-2、MMP-9、TIMP-1、TIMP-2 mRNA相对表达量和TIMP-1、TIMP-2蛋白相对表达水平高于Hp阴性食管癌患者(P<0.05),MMP-2、MMP-9蛋白相对表达水平低于Hp阴性食管癌患者(P<0.05)。结论Hp感染促进食管癌细胞MMPs/TIMPs相关mRNA的转录水平,可正向干预TIMP蛋白表达,但对MMP蛋白表达存在负干预。OBJECTIVE To explore the expression levels of matrix metalloproteinases(MMPs)and Tissue inhibitor of matrix metalloproteinases(TIMPs)in patients with esophageal cancer and analyze their association with Helicobacter pylori infection so as to provide scientific basis for study of nosetiology of esophageal cancer and targeted therapy.METHODS Totally 50 patients with esophageal cancer and 50 patients with esophagitis who were treated in Suining Central Hospital from Jun 2020 to May 2022 were respectively assigned as the esophageal cancer group and the esophagitis group,meanwhile,50 people who received physical examination were chosen as the control group.The Hp infection was detected,and the relative expression levels of MMPs/TIMPs mRNA and proteins were detected by real-time fluorescent quantitative polymerase chain reaction(PCR).RESULTS The positive rate of Hp was 14.0%in the control group,36.0%in the esophagitis group,84.0%in the esophageal cancer group.The relative expression levels of MMP-2,MMP-9,TIMP-1,TIMP-2 mRNA and proteins of the esophagitis group and the esophageal cancer group were higher than those of the control group(P<0.05);The relative expression levels of MMP-2,MMP-9,TIMP-1,TIMP-2 mRNA and proteins of the esophageal cancer group were higher than those of the esophagitis group(P<0.05).In the esophageal cancer group,the relative expression levels of MMP-2,MMP-9,TIMP-1,TIMP-2 mRNA and TIMP-1,TIMP-2 proteins of the Hp-positive patients were higher than those of the Hp-negative patients(P<0.05);the relative expression levels of MMP-2 and MMP-9 proteins of the Hp-positive patients were lower than those of the Hp-negative patients(P<0.05).CONCLUSION The Hp infection may promote the transcription levels of MMPs/TIMPs mRNA in esophageal cancer cells,it positively interferes with the expression of TIMP protein and negatively interferes with the expression of MMP protein.
