从经络辨证出发探讨针刺治疗脑性瘫痪临床选穴思路

摘　　要：小儿脑瘫是儿童时期最常见身体残疾的重要原因之一,其早期诊疗难度大,恢复难度大,是医学界关注的热点。针刺作为中医特色疗法,在治疗脑性瘫痪方面有健全的理论体系和丰富的临床手段。该文将针刺治疗该病的相关研究从循经取穴、经验取穴、针刺方法等方面归类整理,归纳总结临床治疗思路、临床疗效评价以及不同针刺方案治疗脑性瘫痪的作用机制,探讨趋势,分析规律,以期对小儿脑性瘫痪提供最佳的临床治疗思路。通过研究发现脑性瘫痪的针刺治疗应动态地分析证候规律,把握其阶段性特征和发展趋向,规范和补充中医辨证论治方法,尤其是发挥经络辨证在不同分型、不同阶段治疗中的优势,重视头针修复中枢性损伤状态的作用、督脉夹脊穴改善核心稳定性和平衡运动能力的作用,根据脑性瘫痪的类型、阶段临床特点选取不同的阴阳经组穴方案、针刺方法以提高临床疗效。Pediatric cerebral palsy is one of the most common and important causes of physical disability in childhood,and its early diagnosis and treatment is difficult and recovery is difficult,which is a hot spot of concern in the medical field.Acupuncture,as a characteristic therapy of Chinese medicine,has a sound theoretical system and abundant clinical means in the treatment of cerebral palsy.This paper categorizes and organizes the studies related to acupuncture treatment of this disease in terms of meridiantracking,empirical acupuncture points and acupuncture methods,summarizes the clinical treatment ideas,clinical efficacy evaluation and the mechanism of action of different acupuncture protocols in the treatment of cerebral palsy,explores the trends and analyzes the patterns,in order to provide the best clinical treatment ideas for pediatric cerebral palsy.Through the study,we found that the acupuncture treatment of cerebral palsy should dynamically analyze the pattern of evidence,grasp its stage characteristics and developmental tendency,standardize and supplement the method of TCM evidence-based treatment,especially the advantages of meridian evidence-based treatment in different subtypes and stages,pay attention to the role of head acupuncture in repairing the central damage state,and the role of the Du channel Jiaji(EX-B2)in improving core stability and balance movement ability.Selecting different acupuncture points and acupuncture methods according to the type and the phased clinical characteristics of cerebral palsy in order toto improve clinical efficacy.
