摘 要:目的探讨龋病患儿唾液中分泌型免疫球蛋白A(sIgA)、基质金属蛋白酶(MMPs)的变化与唾液中变形链球菌、远缘链球菌检出水平的相关性。方法回顾性选取2019年7月至2021年7月自贡市第一人民医院口腔门诊确诊的重度龋病患儿60例(重度组)、轻度龋病患儿60例(轻度组)、无龋病患儿60例(对照组)。采用酶联免疫吸附试验检测3组患儿唾液中sIgA、MMPs水平,采用实时单量PCR技术检测3组儿童唾液中变形链球菌、远缘链球菌水平,并分析sIgA、MMPs与变形链球菌、远缘链球菌的相关性。结果重度组患儿唾液中sIgA、MMP-2、MMP-9水平均分别高于对照组,轻度组sIgA、MMP-9水平分别高于对照组,重度组患儿唾液中sIgA、MMP-2水平均分别高于轻度组,差异均有统计学意义(P<0.05)。重度组患儿唾液中变形链球菌、远缘链球菌水平均分别高于轻度组、对照组,轻度组唾液中变形链球菌、远缘链球菌水平分别高于对照组,差异均有统计学意义(P<0.05)。经线性相关法分析,龋病患儿唾液中变形链球菌、远缘链球菌、sIgA、MMP-2、MMP-9水平与龋失补指数均呈正相关关系(r=0.538、0.462、0.668、0.611、0.582,P<0.05)。经多元线性回归法分析,唾液中变形链球菌、远缘链球菌、sIgA、MMP-2、MMP-9水平增加会增大儿童罹患龋齿的风险(P<0.05)。结论唾液中变形链球菌、远缘链球菌、sIgA、MMP-2、MMP-9水平增加与儿童罹患龋齿有关,并且与龋病病情程度呈正相关关系。Objective To investigate the relationship between the changes of salivary immunoglobulin A(sIgA)and matrix metalloproteinases(MMPs)in saliva and the detection levels of Streptococcus mutans and Streptococcus sobrinus in saliva.Methods Sixty children(severe group)with severe rickets diagnosed in The First People's Hospital of Zigong City from July 2019 to July 2021,60 children with mild rickets(mild group)and 60 children without rickets(control group).ELISA was used to detect the levels of sIgA and MMPs in saliva of three groups of children.The levels of Streptococcus mutans and Streptococcus sobrinus in saliva of three groups of children were detected using real-time single quantity PCR technology,and the correlation between sIgA,MMPs,Streptococcus mutans,and Streptococcus sobrinus was analyzed.Results After analysis,the levels of sIgA,MMP-2 and MMP-9 in the saliva of the severe group were higher than those in the control group(P<0.05),and the levels of sIgA and MMP-9 in the mild group were higher than those in the control group(P<0.05).The levels of sIgA and MMP-2 in the saliva of the children were higher than those in the mild group(P<0.05).The levels of Streptococcus mutans and Streptococcus mutans in the saliva of the severe group were higher than those in the mild group and the control group(P<0.05),the levels of Streptococcus mutans and Streptococcus sobrinus in the saliva of the mild group were higher than those in the control group(P<0.05).According to the linear correlation analysis,Streptococcus mutans,Streptococcus sobrinus,sIgA,saliva in children with rickets.The levels of MMP-2 and MMP-9 were positively correlated with the sputum loss index(P<0.05).Multiple linear regression analysis showed that elevated levels of Streptococcus mutans,Streptococcus sobrinus,sIgA,MMP-2,and MMP-9 in saliva increased the risk of dental caries in children(P<0.05).Conclusion Increased levels of Streptococcus mutans,Streptococcus sobrinus,sIgA,MMP-2,and MMP-9 in saliva are associated with caries in children and are positiv
