摘  要:目的探讨中国版甲状腺影像报告和数据系统(C-TIRADS)联合BRAF^(V600E)检测对非典型结节/无明确意义滤泡样损伤(AUS/FLUS)甲状腺结节良恶性的诊断效能。方法选取100例AUS/FLUS甲状腺结节患者,患者接受甲状腺超声检查和BRAF^(V600E)基因突变检测,分析C-TIRADS、BRAF^(V600E)基因诊断单用及联合使用对甲状腺结节良恶性的鉴别诊断效能。结果C-TIRADS分类诊断出恶性结节71例、良性29例。BRAF^(V600E)基因突变检测显示阳性63例,阴性37例。C-TIRADS联合BRAF^(V600E)基因突变诊断阳性82例,阴性18例。与C-TIRADS、BRAF^(V600E)单用相比,两项联合对甲状腺结节的诊断灵敏度(98.78%)、准确度(96.00%)及阴性预测值(88.89%)更高(P<0.05);与C-TIRADS相比,两项联合对甲状腺恶性结节的特异度(88.89%)更高(P<0.05)。结论C-TIRADS分类方法对甲状腺结节良恶性判定有较高的诊断效能,联合BRAF^(V600E)基因突变检测可进一步提高恶性甲状腺结节的诊断灵敏度及准确度。Objective To explore the diagnostic efficacy of the Chinese version of thyroid imaging reporting and data system(C-TIRADS)combined with BRAF^(V600E) detection in the diagnosis of benign and malignant thyroid nodules with atypical nodules/unspecified follicular lesions(AUS/FLUS).Methods We selected 1oo patients with AUS/FLUS thyroid nodules,who received thyroid ultrasound examination and BRAF^(V600E) gene mutation detection,and analyzed the differential diagnosis efficacy of C-TIRADS and BRAF^(V600E) gene diagnosis alone and in combination for benign and malignant thyroid nodules.Results C-TIRADS clssified 71 malgnant nodules and 29 benign nodules.BRAF^(V600E) gene mutation detection showed that 63 cases were positive and 37 cases were negative C-TIRADS combined with BRAF^(V600E) gene mutation diagnosis was positive in 82 cases and negative in 18 cases.Compared with C-TIRADS and BRAF^(V600E) alone,the sensitivity(98.78%),acuracy(96.00%)and negative predictive value(88.89%)of the combination of the two methods for thyroid nodules were higher(P<0.05).Compared with C-TIRADS,the specificity(88.89%)of the combination of the two methods for thyroid malignant nodules was higher(P<O.05).Conclusions C-TIRADS classification method has high diagnostic efficiency in the determination of benign and malignant thyroid nodules.combined with BRAF^(V600E) gene mutation detection can further improve the diagnostic sensitivity and accuracy of malignant thyroid nodules.
