腹腔镜联合超声引导下经皮微波消融子宫腺肌病中长期疗效分析

摘　　要：目的 分析腹腔镜联合超声引导下经皮微波消融子宫腺肌病的中长期疗效。方法 132例子宫腺肌病患者在腹腔镜联合超声引导下行经皮穿刺微波消融治疗,并完成了3年随访。记录患者术前及术后3个月、6个月、1年、2年、3年子宫体积、腺肌瘤体积、痛经评分、月经量评分、症状严重程度评分和健康相关生活质量评分,连续比较分析近期(术后3个月)、中长期(术后6个月~3年)临床疗效情况,并记录术后并发症。结果 与术前相比,术后3个月、6个月、1年、2年、3年子宫体积及子宫腺肌瘤体积明显缩小,痛经评分、月经量评分及症状严重程度评分明显下降,健康相关生活质量评分明显升高(P<0.05),差异有统计学意义。术后无严重并发症。结论 腹腔镜联合超声引导下经皮微波消融子宫腺肌病安全有效,中长期疗效确切。Objective To evaluate the medium and long-term clinical efficacy of laparoscopy and sonographyguided percutaneous microwave ablation for adenomyosis.Methods A total of 132 patients with adenomyosis underwent laparoscopy and sonography-guided percutaneous microwave ablation,and were followed up regularly for 3 years.The dysmenorrhea severity pain score,menstrual volume score,symptom severity score,health-related quality of life score and the uterine and lesion volume were recorded before ablation and 3 months,6 months,1 year,2 years,3 years after ablation and short-term(3 months after ablation)and medium-and long-term(6 months to 3 years after ablation)clinical efficacy were continuously compared and analyzed,and postoperative complications were recorded.Results The dysmenorrhea severity pain score,menstrual volume score,symptom severity score and the uterine and lesion volume all decreased significantly and the health-related quality of life score increased significantly at 3 months,6 months,1 year,2 years,3 years after ablation compared with the baseline before ablation,and the differences were statistically significant(P<0.05).There was no serious complication after ablation.Conclusions Laparoscopy and sonography-guided percutaneous microwave ablation is a safe and effective treatment for adenomyosis with difinite medium and long-term clinical efficacy.
