摘　　要：目的探讨前列腺癌患者外周血中的外泌体AR-V7 mRNA表达与新型内分泌治疗的敏感时间的相关性。方法选取2018年1月至2020年6月在本院收治的36例前列腺癌患者的临床资料,检测其外周血中的外泌体AR-V7 mRNA表达并随访其新型内分泌治疗的敏感时间。分析患者的临床资料、AR-V7 mRNA表达与新型内分泌治疗的敏感时间的相关性。结果Gleason评分、N分期和外周血中的AR-V7表达是影响新型内分泌治疗的敏感时间的因素(P=0.101、0.062、0.028)。Cox多因素回归分析结果提示N分期、外周血中的AR-V7表达是前列腺癌新型内分泌治疗的敏感时间的独立预测因素(HR=1.696、2.990,95%CI:1.102~2.610、1.164~7.683,P=0.016、0.023)。AR-V7阳性组的新型内分泌治疗的中位敏感时间是(16.4±2.2)个月,而AR-V7阴性组为(22.1±1.9)个月,差异有统计学意义(P=0.028)。结论N分期及外周血中的外泌体AR-V7表达与前列腺癌患者行新型内分泌治疗的敏感时间存在相关性,AR-V7阳性表达患者的敏感时间较阴性表达者短。Objective To investigate the correlation between the expression of exosomal AR-V7 mRNA in peripheral blood of prostate cancer patients and the response time to new endocrine therapy.Methods The clinical data of 36 patients with prostate cancer admitted to our hospital from January 2018 to June 2020 were selected.The expression of exosomal AR-V7 mRNA in peripheral blood was detected,and the sensitivity time of new endocrine therapy was followed up.The clinical data,AR-V7 mRNA expression and the time to response to new endocrine therapy were statistically analyzed.Results Gleason score,N stage and AR-V7 expression in peripheral blood were the factors affecting the response time to new endocrine therapy(P=0.101,0.062,0.028).Multivariate regression analysis showed that N stage and AR-V7 expression in peripheral blood were independent predictors of response to novel endocrine therapy for prostate cancer(HR=1.696,2.990,95%CI:1.102-2.610,1.164-7.683,P=0.016,0.023).The median response time to new endocrine therapy in AR-V7 positive group was(16.4±2.2)months,while that in AR-V7 negative group was(22.1±1.9)months,and the difference was statistically significant(P=0.028).Conclusions The N stage and the expression of exosomal AR-V7 in peripheral blood are correlated with the sensitivity time of new endocrine therapy in prostate cancer patients.The sensitivity time of patients with positive expression of AR-V7 is shorter than that of patients with negative expression.
