腹腔镜异位嗜铬细胞瘤切除术的安全性和疗效分析

摘　　要：目的探讨腹腔镜异位嗜铬细胞瘤切除术的安全性和疗效。方法回顾性分析2010年10月至2020年10月在本院行嗜铬细胞瘤切除术的112例患者的临床资料,采用开放肾上腺嗜铬细胞瘤切除术的患者未纳入研究。按照肿瘤位置及手术方式将患者分为腹腔镜异位嗜铬细胞瘤切除组(LEAP组,18例)、开放异位嗜铬细胞瘤切除组(OPEN组,20例)和腹腔镜下肾上腺嗜铬细胞瘤切除组(LAP组,74例),比较各组手术相关及预后指标。结果LEAP组的手术时间较OPEN组更短(P<0.001),术中估算失血量更少(P=0.012),术后住院时间更短(P<0.001),肿瘤最长径更小(P=0.002)。LEAP组的术中估算出血量较LAP组更多(P=0.018),重症监护病房的住院时间更长(P=0.040),术中最高收缩压更高(P=0.001)。共有7例患者术后发生并发症,组间比较差异无统计学意义(P>0.05)。结论腹腔镜异位嗜铬细胞瘤切除术和腹腔镜下肾上腺嗜铬细胞瘤切除术的安全性和疗效接近,腹腔镜异位嗜铬细胞瘤切除术对于经验丰富的外科医生是一种可行的、可重复操作的手术方式。Objective To investigate the safety and efficacy of laparoscopic resection of ectopic pheochromocytoma.Methods The clinical data of 112 patients who underwent pheochromocytoma resection in our hospital from October 2010 to October 2020 were retrospectively analyzed.Patients who underwent open adrenal pheochromocytoma resection were not included in the study.According to the tumor location and surgical method,the patients were divided into laparoscopic resection of ectopic pheochromocytoma group(LEAP group,18 cases),OPEN resection of ectopic pheochromocytoma group(OPEN group,20 cases)and laparoscopic resection of adrenal pheochromocytoma group(LAP group,74 cases).The operation related and prognostic indicators of each group were compared.Results Compared with OPEN group,LEAP group had shorter operative times(P<0.001),less estimated blood loss(P=0.012),shorter postoperative recovery(P<0.001),and smaller tumor size(P=0.002).Compared with the LAP group,the LEAP group had more estimated blood loss(P=0.018)and longer intensive care unit stay(P=0.40).The highest intraoperative systolic blood pressure was higher(P=0.001).A total of 7 cases of patients with postoperative complications,there was no statistically significant difference between groups compare(P>0.05).Conclusions The safety and efficacy of laparoscopic resection of ectopic pheochromocytoma and laparoscopic resection of adrenal pheochromocytoma are similar.Laparoscopic resection of ectopic pheochromocytoma is a feasible and reproducible surgical method for experienced surgeons.
