同心双焦软性角膜接触镜的短期临床有效性及满意度分析

摘　　要：目的研究分析近视儿童青少年配戴同心双焦软性角膜接触镜的短期临床有效性及临床满意度。方法选取2022年5~12月在首都医科大学附属北京儿童医院眼科就诊并完成3个月随访的近视儿童青少年88例(均采用右眼数据,共88眼),其中同心双焦软性角膜接触镜(简称双焦软镜)组30例(30眼),角膜塑形镜组30例(30眼),单焦点框架眼镜(简称普通框镜)组28例(28眼)。所有患者在戴镜前和戴镜3个月后均进行眼轴测量,以眼轴数据作为短期有效性观察指标,并在戴镜3个月后对双焦软镜组儿童监护人进行临床满意度问卷调查。采用Kruskal-Wallis检验及Bonferroni校正的正态近似法对3组眼轴变化数据进行统计学分析。结果3组近视儿童青少年短期内眼轴均有增长,双焦软镜组、角膜塑形镜组及普通框镜组眼轴长度变化量(增量)分别为(0.03±0.05)mm、(0.04±0.05)mm及(0.18±0.19)mm,普通框镜组高于双焦软镜组和角膜塑形镜组,差异均有统计学意义(P<0.05),双焦软镜组和角膜塑形镜组接近,差异无统计学意义(P>0.05)。对双焦软镜组临床满意度调查分析显示,患儿视觉质量清晰度及舒适度满意占比90%以上。结论配戴双焦软镜在短期内对近视儿童青少年眼轴增长具有良好的控制效果及临床满意度,为儿童青少年早期近视防控提供有效手段。Objective To investigate the short-term clinical effectiveness of concentric bifocal soft contact lenses on myopia control and its clinical satisfaction in myopic children and adolescents.Methods Retrospective case series studies.A total of 88 cases(88 eyes,using the data of the right eye)with myopia were enrolled in this study.They were treated in the Department of Ophthalmology of Beijing Children's Hospital,Capital Medical University from May 2022 to December 2022 and followed up for 3 months.All the cases were divided into three groups:concentric bifocal soft contact lens group(referred to as bifocal soft lens group,30 cases,30 eyes),orthokeratology group(30 cases,30 eyes)and single focus frame glasses group(referred to as ordinary frame glasses group,28 cases,28 eyes).Binocular axial lengths were measured as the shortterm effectiveness observation index in all patients before and after wearing the glasses for 3 months.Clinical satisfaction questionnaire was conducted for the children guardians in the bifocal soft contact lens group after treatment for 3 months.The Kruskal-Wallis test was used to statistically analyze the axial length changes in three groups.Results The axial length of all myopic children and adolescents in the three groups increased in the short term.The increase in axial length in the bifocal soft contact lens group,the orthokeratology group and the ordinary frame glasses group was(0.03±0.05)mm,(0.04±0.05)mm and(0.18±0.19)mm,respectively.The increase in axial length in the ordinary frame glasses group were higher than those in the concentric bifocal soft contact lens group and orthokeratology lens group,and the differences were statistically significant(P<0.05).The increases of the axial length were close between the concentric bifocal soft contact lens group and orthokeratology lens group,and the difference was not statistically significant(P>0.05).According to the survey and analysis of the clinical satisfaction in the concentric bifocal soft contact lens group,the proportion of childr
