高速涡轮机头联合微创拔牙刀在下颌阻生牙拔除术患者中的应用效果

摘　　要：目的:观察高速涡轮机头联合微创拔牙刀在下颌阻生牙拔除术患者中的应用效果。方法:选取2019年6月至2021年6月该院收治的行下颌阻生牙拔除术的80例患者进行前瞻性研究,按照随机数字表法分为对照组和观察组各40例。对照组给予凿骨劈冠拔除术,观察组给予高速涡轮机头联合微创拔牙刀拔除术,比较两组手术时间、患牙牙窝完整性评分、术后疼痛程度[视觉模拟评分法(VAS)]、并发症发生率和治疗满意度。结果:观察组手术时间短于对照组,牙窝完整性评分高于对照组,差异均有统计学意义(P<0.05);术后1、7 d,观察组VAS评分均低于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率为7.50%,低于对照组的25.00%,差异有统计学意义(P<0.05);观察组治疗满意度为97.50%,高于对照组的80.00%,差异有统计学意义(P<0.05)。结论:高速涡轮机头联合微创拔牙刀应用于下颌阻生牙拔除术患者可缩短手术时间,降低VAS评分和术后并发症发生率,提高牙窝完整性评分和治疗满意度,优于凿骨劈冠拔除术治疗效果。Objective:To observe application effects of high-speed turbine head combined with minimally invasive tooth extraction knife in patients undergoing mandibular impacted tooth extraction surgery.Methods:A prospective study was conducted on 80 patients who underwent mandibular impacted tooth extraction from June 2019 to June 2021.According to the random number table method,they were divided into control group and observation group,40 cases in each group.The control group was treated with chisel split crown extraction,while the observation group was treated with high-speed turbine head combined with minimally invasive tooth extraction knife extraction.The operation time,the dental fossa integrity score,the pain degree[visual analogue scale(VAS)],the postoperative complication rate,and the treatment satisfaction were compared between the two groups.Results:The operation time of the observation group was shorter than that of the control group,the dental fossa integrity score was higher than that of the control group,and the differences were statistically significant(P<0.05).1 and 7 days after the operation,the VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).The postoperative complication rate in the observation group was 7.50%,which was lower than 25.00%in the control group,and the difference was statistically significant(P<0.05).Further,the treatment satisfaction of the observation group was 97.50%,which was higher than 80.00%of the control group,and the difference was statistically significant(P<0.05).Conclusions:The high-speed turbine head combined with minimally invasive tooth extraction knife in the patients undergoing mandibular impacted tooth extraction can shorten the operation time,reduce the VAS score and the postoperative complication rate,and improve the dental fossa integrity score and the treatment satisfaction.Moreover,it is superior to chisel split crown extraction.
