过敏性结膜炎患儿血清与泪液中HA、TSLP、ECP水平变化特点分析

摘　　要：目的探究过敏性结膜炎(AC)患儿血清、泪液中透明质酸(HA)、胸腺基质淋巴细胞生成素(TSLP)和嗜酸性细胞阳离子蛋白(ECP)水平变化特点。方法选取2020年6月—2022年6月本院收治的AC患儿113例为观察组,根据患儿AC分型分为季节性AC组(SAC组,n=42)、常年性AC组(PAC组,n=38)和春季角膜炎组(VKC组,n=33),另选取同期体检的健康儿童35例为对照组,比较各组血清、泪液中HA、TSLP、ECP水平,分析HA、TSLP、ECP水平与IgE的相关性。结果观察组患儿血清和泪液中HA、TSLP、ECP表达水平均明显高于对照组(P<0.05)。观察组中,不同严重程度患儿的血清和泪液中HA、TSLP和ECP表达水平比较,轻度<中度<重度(P<0.05);VKC组血清TSLP、ECP水平和泪液TSLP水平均明显高于SAC组、PAC组(P<0.05),差异有统计学意义;PAC组泪液ECP水平均明显高于SAC组、VKC组(P<0.05);SAC组、PAC组和VKC组患儿的血清、泪液HA水平比较,差异无统计学意义(P>0.05)。治疗后,观察组患儿的血清和泪液HA、TSLP、ECP表达水平均较治疗前明显下降,差异有统计学意义(P<0.05)。结论AC患儿存在血清和泪液中HA、TSLP、ECP表达水平升高的现象,且患儿病情越严重,其表达水平越高;不同类型AC患儿血清和泪液中HA、TSLP、ECP表达水平存在差异,可为临床AC发病机制及疾病诊断的研究提供思路。Objective This paper aims to explore the changes of hyaluronic acid(HA),thymic stromal lymphopoietin(TSLP)and eosinophil cationic protein(ECP)levels in serum and tears of children with allergic conjunctivitis(AC).Methods A total of 113 children with AC admitted to Henan Children's Hospital were selected as the observation group between June 2020 and June 2022.According to AC typing,the children enrolled were divided into seasonal AC group(SAC group,n=42),perennial AC group(PAC group,n=38)and vernal keratitis group(VKC group,n=33).Meanwhile,35 healthy children were selected as the control group.The levels of HA,TSLP and ECP in serum and tears were compared among the groups.The correlation between IgE and the levels of HA,TSLP and ECP was analyzed.Results The levels of HA,TSLP and ECP in serum and tears of the observation group were significantly higher than those in the control group(P<0.05).In the observation group,the levels of HA,TSLP and ECP in serum and tears increased with the severity of AC(mild<moderate<severe),with the differences statistically significant(P<0.05).The levels of serum TSLP and ECP and the TSLP level in tear in the VKC group were significantly higher than those in the SAC group and PAC group(P<0.05).ECP level in tear in the PAC group was significantly higher than that in the SAC group and the VKC group(P<0.05).The difference of HA level in serum and tears showed no statistical significance among the SAC group,the PAC group and the VKC group(P>0.05).After treatment,the levels of HA,TSLP and ECP in serum and tears of the observation group were significantly reduced,with the differences statistically significant(P<0.05).Conclusion The levels of HA,TSLP and ECP are elevated in serum and tears of children with AC.The severer the children's condition,the higher the levels.The levels of HA,TSLP and ECP in serum and tears are different in children with different types of AC,which devotes to the study of clinical pathogenesis and diagnosis of AC.
