前列腺癌患者手术前后血清FPSA/TPSA、PSMA水平检测对预后评估的价值

摘　　要：目的探讨前列腺癌患者在接受手术治疗前后,对其游离的前列腺癌特异性抗原(FPSA)、总前列腺癌特异性抗原(TPSA)、FPSA/TPSA、前列腺特异性膜抗原(PSMA)水平进行检测的分析指标对于评价患者预后效果的作用。方法将40例健康体检者作为参照组,60例前列腺癌患者作为研究组,比较2组血清指标检测结果,对研究组在不同GS评分及不同预后情况下血清检测指标变化情况进行对照分析,并进行ROC曲线价值分析。结果研究组患者PSMA、FPSA、TPSA均较参照组高,FPSA/TPSA水平较参照组低,差异均具有统计学意义(P<0.05);GS≤7分患者和>7分患者的PSMA、FPSA、TPSA、FPSA/TPSA水平差异有统计学意义(P<0.05);血清FPSA、TPSA、PSMA、FPSA/TPSA预测预后的曲线下面积分别为0.800、0.791、0.945、0.988,均有预测价值。结论血清FPSA/TPSA、PSMA水平检测在前列腺癌患者手术治疗前后有重要的应用价值,能够有效评价患者病情严重程度,评估预后效果。Objective This paper aims to investigate the detection results of serum indexes in patients with prostate cancer before and after surgical treatment,including free prostate specific antigen(FPSA),total prostate specific antigen(TPSA),FPSA/TPSA and prostate specific membrane antigen(PSMA).The role of these indicators in evaluating the prognosis of patients was analyzed.MethodsA total of 40 healthy people were selected as the reference group and 60 patients with prostate cancer were selected as the study group.The results of serum indexes in the two groups were compared,and the changes of serum indexes in the study group under different CS scores and different prognosis were compared and analyzed,and the ROC curve value analysis was also performed.Results PSMA,FPSA and TPSA of the study group were higher than those of the reference group,the level of FPSA/TPSA was lower than that of the reference group,and the differences were statistically significant(P<0.05).The differences of PSMA,FPSA,TPSA,FPSA/TPSA levels in the patients with GS≤7 and the patients with GS>7 were statistically significant(P<0.05).The area under the curve of FPSA,TPSA,PSMA,and FPSA/TPSA in the prediction of prognosis was respectively 0.800,0.791,0.945 and 0.988,suggesting that they all had predictive value.Conclusion The detection of FPSA/TPSA and PSMA levels has important application value for patients with prostate cancer before and after surgical treatment.It can effectively evaluate patients'condition and the prognosis.
