摘  要:光动力治疗具有毒副作用小、适用瘤种广和可反复治疗等优势,在临床上已用于食管癌、中央型肺癌、脑胶质瘤等的根治、辅助或姑息治疗。光动力治疗除了通过产生活性氧,直接杀伤肿瘤细胞外,还可产生肿瘤原位疫苗,引起肿瘤的免疫原性细胞死亡,促进树突状细胞的成熟和细胞毒性T细胞的浸润,具有一定免疫治疗的属性。但光动力治疗存在氧气依赖和光敏剂的肿瘤选择性不足等缺陷,且单独光动力治疗诱导的弱免疫不足以触发全身抗肿瘤免疫反应。本文重点介绍光动力免疫治疗的机制,并针对上述不足阐述提高光动力免疫治疗疗效的策略。Photodynamic therapy(PDT)has the advantages of limited side effects,wide range of applicable tumors and repeated treatment.It has been used for radical,adjuvant or palliative treatment of esophageal cancer,central lung cancer,and glioma in clinical practice.PDT,with certain attributes of immunotherapy,can not only kill tumor cells directly by producing reactive oxygen species,but also produce in situ cancer vaccine and induce immunogenic cell death of tumors,promoting dendritic cell maturation and cytotoxic T cell infiltration.However,PDT has defects such as oxygen dependence and insufficient tumor selectivity of photosensitizers,and immunity induced by PDT alone is too weak to trigger systemic anti-tumor immune responses.This article will focus on the mechanism of photodynamic immunotherapy(PDIT),and elaborate strategies to improve the efficacy of PDIT according to the above shortcomings.
