早期食管癌术后肿瘤复发的影响因素分析

摘　　要：目的分析早期食管癌术后肿瘤复发的影响因素。方法回顾性分析60例早期食管癌患者的临床资料,依据术后肿瘤复发情况分为对照组(未复发,35例)与观察组(复发,25例)。对比两组临床资料,并采用Logistic回归分析早期食管癌术后肿瘤复发的影响因素。结果观察组患者的肿瘤直径≥5 cm占比64.00%、肿瘤浸润深度T1b占比56.00%均高于对照组的40.00%、28.57%,病灶完整切除率56.00%、术后参与化疗率60.00%低于对照组的80.00%、94.29%,差异有统计学意义(P<0.05);两组患者的年龄≥55岁占比、肿瘤位置、手术方式对比差异无统计学意义(P>0.05)。经Logistic回归分析显示,肿瘤直径、病灶完整切除、肿瘤浸润深度、术后参与化疗均为早期食管癌术后肿瘤复发的影响因素(P<0.05)。结论肿瘤直径、病灶完整切除、肿瘤浸润深度、术后参与化疗均为早期食管癌术后肿瘤复发的影响因素。Objective To analyze the influencing factors of tumor recurrence after surgery for early esophageal cancer.Methods Clinical data of 60 patients with early esophageal cancer were retrospectively analyzed and divided into control group(no recurrence,35 cases)and observation group(recurrence,25 cases)according to postoperative tumor recurrence.The clinical data of the two groups was compared,and Logistic regression was used to analyze the influencing factors of tumor recurrence after surgery for early esophageal cancer.Results The tumor diameter≥5 cm accounted for 64.00% and stage T1b depth of tumor invasion accounted for 56.00% in the observation group,which were higher than those of 40.00%and 28.57% in the control group;the rate of complete lesion resection of 56.00%and postoperative chemotherapy participation rate of 60.00% in the observation group were lower than those of 80.00% and 94.29% in the control group;the differences were statistically significant(P<0.05).There was no statistically significant difference between the two groups in terms of the percentage of patients aged≥55 years,tumor location,and surgical approach(P>0.05).Logistic regression analysis showed that tumor diameter,complete lesion resection,depth of tumor invasion,and postoperative chemotherapy were all influencing factors of tumor recurrence after surgery for early esophageal cancer(P<0.05).Conclusion The tumor diameter,complete lesion resection,depth of tumor invasion,and postoperative chemotherapy are influencing factors of tumor recurrence after surgery for early esophageal cancer.
