非经典抗精神病药奥氮平致下肢深静脉血栓形成1例患者的药学监护

摘　　要：目的为非经典抗精神病药——奥氮平致下肢深静脉血栓形成患者制定个体化、精准化治疗方案提供参考借鉴。方法2022年9月4日该院收治奥氮平致下肢深静脉血栓形成患者1例,对其进行医院内全程的药学监护,包括手术前后药物使用、抗凝溶栓药物合理使用、用药安全教育、与临床医师共同负责特殊患者药物使用等,并对患者出院后进行跟踪随访,协助医师调整用药方案。结果医师、药师、患者三者共同参与的个体化用药方案与医师的手术治疗方案相结合取得满意的临床效果,且用药安全、有效,整个药事服务过程环节简单、高效。结论药学监护的实施保障患者手术前后药物抗凝治疗的安全性,使临床监测指标达标并维持稳态,发挥了药学服务的积极有益性,同时,提高了相关临床第一线药物的合理使用率。Objective To provide reference for formulating the individual and precise treatment scheme in the patients with deep vein thrombosis of lower limbs caused by non-classic antipsychotic drug olanzapine.Methods One case of lower limb deep vein thrombosis caused by olanzapine was treated in this hospital on September 4,2022,conducted the whole process intra-hospital phamaceutical monitroing,including preoperative and postoperative drug use,rational use of anticoagulant and thrombolytic drugs,medication safe education and working with the clinicians on special patient medication,etc.,conducted the track follow up on the patient after discharge from hospital,and assisted the physician to adjust medication scheme.Results The individualized medication scheme jointly participated by the doctors,pharmacists and patients combined with the doctor′s surgical treatment plan achieved the satisfactory clinical results,moreover the medication was safe and effective.The whole pharmaceutical service process was simple and efficient.Conclusion The implementation of pharmaceutical care ensures the safety of the patient′s anticoagulant treatment before and after surgery,makes the clinical monitoring indicators to reach the standard and maintain the stable state,gives play to the positive benefits of pharmaceutical services,meanwhile improves the rational use rate of related clinical first-line drugs.
