柴贝止痫汤联合抗癫痫药物治疗癫痫疗效的Meta分析

摘　　要：目的:系统评价柴贝止痫汤联合抗癫痫药物(AEDs)治疗癫痫的临床疗效。方法:计算机检索建库以来中国知网、万方数据库、维普数据库、中国临床试验注册中心、中国生物医学文献数据库、PubMed、the Cochrane Library、EMbase、MedLine以及Web of Science。检索以柴贝止痫汤联合西药治疗癫痫的随机对照试验,按照纳入标准及排除标准筛选文献、提取有效资料,评价所有纳入研究的方法学质量,采用RevMan 5.4.1以及Stata 12软件进行Meta分析。结果:共纳入7篇文献,涉及病人555例,其中试验组275例,对照组280例。Meta分析结果显示,试验组临床有效率[RR=1.49,95%CI(1.29,1.72),P<0.00001]、中医证候改善率[RR=4.94,95%CI(2.80,8.71),P<0.00001]高于对照组,差异均有统计学意义;试验组中医症状积分低于对照组,差异有统计学意义[SMD=-1.55,95%CI(-1.96,-1.13),P<0.00001],脑电图改善程度优于对照组,差异有统计学意义[RR=1.72,95%CI(1.17,2.53),P=0.006]。安全性分析显示,试验组8例出现不良反应,对照组5例出现不良反应,两组不良反应发生率比较差异无统计学意义。结论:现有证据表明,柴贝止痫汤联合抗癫痫药物在临床有效率、中医证候改善率、中医症状积分方面均优于单独应用AEDs。Objective:To evaluate the clinical efficacy of Chaibei Zhixian Decoction combined with Anti-epileptic drugs in treating epilepsy.Methods:China National Knowledge Infrastructure(CNKI),WanFang Data,China Science and Technology Journal Database(VIP),Chinese Clinical Trial Registry,China Biology Medicine disc,PubMed,the Cochrane Library,EMbase,MedLine,and Web of Science were searched to collect the randomized controlled trials(RCT)of Chaibei Zhixian Decoction combined with western medicine for epilepsy from the inception of the databases to Octorber 22,2020.After literature screening,data extraction,and quality evaluation,RevMan 5.4.1 software and Stata version 12 software were used for Meta-analysis.Results:A total of 7 literatures were involved,including 555 patients,with 275 cases in experimental group and 280 cases in control group.The results of Meta-analysis showed that the clinical effective rate(RR=1.49,95%CI 1.29-1.72,P<0.00001)and the improvement rate of traditional Chinese medicine(TCM)symptoms(RR=4.94,95%CI 2.80-8.71,P<0.00001)in experimental group were higher than those in control group.The TCM symptom score of experimental group was lower than that of control group(SMD=-1.55,95%CI-1.96--1.13,P<0.00001),and the improvement degree of electroencephalograph(EEG)was better than that of control group(RR=1.72,95%CI 1.17-2.53,P=0.006).There were 8 patients suffered from adverse reactions in experimental group and 5 patients in control group,without significant difference.Conclusion:Existing evidence shows that Chaibei Zhixian Decoction combined with AEDs is superior to AEDs alone in improving the clinical effective rate,TCM syndromes,and TCM symptom scores of epilepsy patients.
