幽门螺杆菌感染对老年消化性溃疡患者血清PGⅠ、PGⅡ、GS、PINP、TNF-α水平的影响

摘　　要：目的探讨幽门螺杆菌感染对老年消化性溃疡患者血清胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、胃泌素(GS)、Ⅰ型胶原氨基端前肽(PINP)、肿瘤坏死因子α(TNF-α)水平的影响。方法本研究选取我院诊治的老年消化性溃疡患者200例,根据幽门螺杆菌感染状况分为阳性组(134例,其中胃溃疡82例、十二指肠溃疡52例)与阴性组(66例,其中胃溃疡40例、十二指肠溃疡26例),采用双抗体夹心酶联免疫吸附法检测血清PGⅠ、PGⅡ、GS、PINP、TNF-α水平。结果幽门螺杆菌感染阳性消化性溃疡患者血清PGⅠ、PGⅡ、GS、TNF-α水平高于阴性患者(P<0.05),PINP低于阳性患者(P<0.05)。胃溃疡及十二指肠溃疡幽门螺杆菌感染阳性患者血清PGⅠ、PGⅡ、GS、TNF-α水平高于阴性患者(P<0.05),PINP低于阳性患者。十二指肠溃疡患者血清PGⅠ、GS高于胃溃疡患者(P<0.05),PINP低于胃溃疡患者(P<0.05),PGⅡ及TNF-α水平比较差异无统计学意义(P>0.05)。结论幽门螺杆菌对老年消化性溃疡患者血清PGⅠ、PGⅡ、GS、PINP、TNF-α水平存在一定影响,阳性患者血清PGⅠ、PGⅡ、GS、TNF-α水平明显升高,PINP水平下降,同时与溃疡部位关系密切。Objective To investigate the effects of Helicobacter pylori infection on serum pepsinogenⅠ(PGⅠ),pepsinogenⅡ(PGⅡ),gastrin(GS),typeⅠcollagen N-terminal propeptide(PINP),tumor necrosis factorα(TNF-α)in elderly patients with peptic ulcer.Methods 200 elderly patients with peptic ulcer were selected and divided into positive group(134 cases,including 82 cases of gastric ulcer and 52 cases of duodenal ulcer)and negative group(66 cases,including 40 cases of gastric ulcer and 26 cases of duodenal ulcer)according to the infection status of Helicobacter pylori.Serum PG Ⅰ,PG Ⅱ,GS,PINP and TNF-αlevel were detected by double antibody sandwich enzyme-linked immunosorbent assay.Results Serum levels of PG ⅠPG Ⅱ,GS and TNF-αin patients with peptic ulcer positive for Helicobacter pylori infection.The level of PINP was higher than that of negative patients(P<0.05)and lower than that of positive patients(P<0.05).Serum PG I,PG Ⅱ,GS,TNF-αin patients with positive Helicobacter pylori infection in gastric ulcer and duodenal ulcer.The level of PINP was higher than that of negative patients(P<0.05),and the level of PINP was lower than that of positive patients;Serum PGⅠ,GS of duodenal ulcer patients were higher than those of gastric ulcer patients(P<0.05),PINP was lower than those of gastric ulcer patients(P<0.05),PGⅠand TNF-αwere no significant difference between the two groups(P>0.05).Conclusion Helicobacter pylori has some effects on serum PGI,PG Ⅱ,GS,PINP and TNF-αin elderly patients with peptic ulcer.The serum levels of PG Ⅰ,PG Ⅱ,GS,TNF-αincreased significantly.The level of PINP decreased significantly,which were closely related to the ulcer site.
