老年患者失眠严重程度及其影响因素分析

摘　　要：目的探讨老年患者失眠严重程度状况及其与睡眠质量、睡眠认知等之间的相关性。方法对727例老年患者采用失眠严重程度指数量表(ISI)评估其失眠严重程度,采用匹兹堡睡眠质量指数量表(PSQI)与睡眠个人信念与态度量表(DBAS-16)评估睡眠质量及睡眠认知。结果老年患者不同失眠严重程度得分与匹兹堡睡眠质量指数量表(PSQI)得分、睡眠个人信念与态度量表(DBAS-16)得分,差异有统计学意义(P<0.05);将老年患者的ISI、PSQI与DBAS-16进行相关性分析,结果显示ISI与PSQI呈正相关,ISI、PSQI与DBAS-16呈现负相关,差异有统计学意义(P<0.01)。结论老年失眠患者多以轻中度失眠为主,罹患高血压、糖尿病等慢性病的老年患者更容易出现睡眠问题,睡眠信念与态度得分越低,则失眠严重程度越重,睡眠质量越差,我们应更加关注伴有慢性病及睡眠认知存在问题老年患者的睡眠状况。Objective To explore the relationship between the severity of insomnia and sleep quality,sleep cognition in elderly patients.Methods The Insomnia Severity Index(ISI),Pittsburgh Sleep Quality Index(PSQI)and sleep personal beliefs and attitudes scale(DBAS-16)were used to assess severity of insomnia,sleep quality and sleep cognition in 727 elderly patients.Results The Insomnia Severity Index(ISI)had significantly higher scores in older patients with hypertension,diabetes,the total PSQI and DBAS-16 score,the difference was significant(P<0.05).ISI was positively correlated with PSQI,while ISI and PSQI were negatively correlated with DBAS-16(P<0.01).Conclusion The elderly patients with insomnia were more likely to have mild to moderate insomnia,and the elderly patients with chronic diseases such as hypertension and diabetes were more likely to have sleep problems.The lower the scores of sleep beliefs and attitudes,the more serious the insomnia and the worse the sleep quality was.More attention should be paid to the sleep status of elderly patients with chronic diseases and sleep cognitive problems.
