百合地黄汤辅助治疗肺癌急性放射性肺损伤的临床观察

摘　　要：目的 观察百合地黄汤辅助治疗肺癌急性放射性肺损伤(RILI)的效果及对血清C反应蛋白(CRP)、转化生长因子-β1(TGF-β1)、肿瘤坏死因子-α(TNF-α)和肺功能的影响。方法 肺癌急性RILI患者102例随机分为两组各51例。对照组予西医常规治疗,观察组予百合地黄汤辅助西医常规治疗,两组疗程均为2周。观察两组临床疗效、中医证候积分、肺功能指标[用力肺活量(FVC)、第1秒用力呼吸容积(FEV1)、FEV1/FVC]、血清CRP、TGF-β1、TNF-α水平。结果 观察组临床总有效率为94.12%,高于对照组的80.39%(P <0.05)。与对照组比较,观察组治疗第3、7、14日的咳嗽、气促、咯痰、乏力、胸痛中医证候评分较低(P <0.05)。两组治疗后血清TNF-α、CRP、TGF-β1水平明显下降(P <0.05),且观察组更低(P <0.05)。与治疗前比较,两组治疗后FVC、FEV1、FEV1/FVC水平明显上升(P <0.05),且观察组更高(P <0.05)。结论 百合地黄汤辅助治疗肺癌急性RILI临床效果显著,可明显降低患者中医证候积分,降低患者血清CRP、TGF-β1水平,改善肺功能。Objective:To explore the effect of Baihe Dihuang Decoction in the adjuvant treatment of acute radiation-induced lung injury(RILI)of lung cancer and its influences on serum C-reactive protein(CRP),transforming growth factor-β1(TGF-β1),tumor necrosis factor-α(TNF-α)and lung function.Methods:A total of 102 patients with acute RILI of lung cancer were enrolled and randomly divided into two groups,with 51 cases in each group.The control group was treated with routine western medicine,and the observation group was treated with Baihe Dihuang Decoction assisted with routine western medicine.All patients were treated for 2 weeks.The clinical curative efect,scores of TCM syndromes,lung function indexes[forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1),FEV1/FVC],serum CRP,TGF-β1 and TNF-αin both groups were observed.Results:The total effective rate was higher in the observation group than the control group(P<0.05).At the 3,7 and 14 d after treatment,scores of TCM syndromes(cough,shortness of breath,expectoration,fatigue,chest pain)were lower in the observation group than the control group(P<0.05).After treatment,levels of serum TNF-α,CRP and TGF-β1 in both groups were significantly decreased(P<0.05),and the results were lower in the observation group than those in the control group(P<0.05).After treatment,levels of FVC,FEV1 and FEV1/FVC in both groups were significantly increased(P<0.05),and the results were higher in the observation group than the control group(P<0.05).Conclusion:The clinical effect of Baihe Dihuang Decoction is significant in the adjuvant treatment of acute RILI of lung cancer,which can significantly reduce scores of TCM syndromes,serum CRP and TCF-β1,and improve lung function.
