摘　　要：目的探讨微小核糖核酸(miR)-1973表达水平与鼻咽癌(NPC)患者远处转移及临床预后的关系.方法选取74例NPC患者,根据初诊有无远处转移分为远处转移组(n=20)和无远处转移组(n=54),同期选取26例鼻咽黏膜慢性炎患者作为对照组.用实时荧光定量RT-PCR方法分别检测远处转移组、无远处转移组及对照组活检组织中miR-1973的表达水平,分析其表达强度与鼻咽癌远处转移及临床预后的关系.结果NPC组织miR-1973表达水平显著高于鼻咽黏膜慢性炎,NPC伴远处转移组miR-1973表达水平较无远处转移组显著升高(P<0.0001).miR-1973表达水平与性别、年龄、T分期、淋巴结转移及临床分期无明显相关性(P>0.05).Keplan-Miere生存曲线检验结果显示,miR-1973高表达组患者3年总生存率远低于低表达组患者(P<0.0001).结论miR-1973在NPC组织中高表达,且与远处转移密切相关,可能作为预测NPC远处转移及临床预后的分子标志物.Objective To investigate the expression level of microRNA(miR)-1973 in nasopharyngeal carcinoma(NPC)and its correlations with distant metastasis and clinical prognosis in NPC patients.Methods Seventy-four patients with NPC were chosen and divided into distant metastasis group(n=20)and no distant metastasis group(n=54)according to the presence or absence of distant metastasis at the initial diagnosis;and 26 patients with chronic inflammation of nasopharyngeal mucosa were selected as the control group at the same period.The expression levels of miR-1973 in the biopsy tissues of the three groups were measured by real-time fluorescence quantitative-PCR(QRT-PCR).The correlations between its expression intensity and distant metastasis and clinical prognosis of nasopharyngeal cancer were analyzed.Results The expression level of miR-1973 in NPC tissue was significantly higher than that of nasopharyngeal mucosal chronic inflammation tissue;and miR-1973 expression level was significantly higher in the NPC with distant metastasis group than that of the non-distant metastasis group(P<0.0001).No significant correlation was observed between miR-1973 expression level and gender,age,T-stage,lymph node metastasis or clinical stage(P>0.05).The results of the Keplan-Miere survival curve test indicated that the 3-year overall survival of patients in the miR-1973 high expression group was significantly lower than that of patients in the low expression group(P<0.0001).Conclusion miR-1973 is expressed highly in NPC tissues and associated strongly with distant metastasis,and may be a molecular marker for predicting distant metastasis and clinical prognosis of NPC.
