摘 要:目的探讨多层螺旋CT(MSCT)对消化道迁徙性异物的诊断价值及发生异物迁徙相关因素的临床分析。方法回顾性分析经CT诊断,消化道内镜和手术证实的34例消化道迁徙性异物患者的临床和影像学资料。34例均行MSCT平扫,扫描图像行1.25mm薄层重建,结合MSCT的多平面重建(MPR)、最大密度投影(MIP)、容积再现(VR)等技术进行CT检查结果与内镜及手术所见对照分析。结果异物种类:鱼刺24例,骨片4例,枣核2例,铁丝2例,牙签1例,牙刷1例。异物位置:完全迁徙10例(首检7例,复检3例),部分迁徙24例。CT定位与内镜及手术所见符合率为100%。结论MSCT及后处理技术对消化道迁徙性异物具有较高的诊断价值,并依据发生异物迁徙的相关因素,制订个性化的诊疗计划,以减少并发症的发生,提高临床治愈率。Objective To explore the diagnostic value of multi spiral computed tomography(MSCT)for migratory foreign bodies in the digestive tract and analyze factors associated with the occurrence of migratory foreign bodies.Methods The clinical and imaging data of 34 patients with digestive migratory foreign bodies confirmed by endoscopy and surgery were analyzed retrospectively.MSCT images were reconstructed in 1.25 mm thin layers,and the reconstruction imaging such as multiplanar reconstruction(MPR),maximum intensity projection(MIP),and volume rendering(VR)of MSCT were used to compare the CT scan results with endoscopy and surgery findings.Results Foreign body type:24 cases of fish spikes,4 cases of bone fragments,2 cases of date pits,2 cases of wire,l case of the toothpick,and 1 case of the toothbrush.Foreign body location:10 cases of complete displacement(7 cases at first examination and 3 cases at reexamination)and 24 cases of partial displacement.The agreement of localization between CT and endoscopy as well as surgery was 1o0%.Conclusion MSCT and post-processing techniques have high diagnostic value for migratory foreign bodies in the digestive tract.It can reduce the occurrence of complications and improve the clinical cure rates to make personalized diagnosis and treatment plans according to the relevant factors of migratory foreign bodies.
