CT血管成像技术对肝泡状棘球蚴病术前第2肝门血管受侵的评价

摘　　要：目的探讨终末期肝泡状棘球蚴病(HAE)患者行自体肝移植前CT血管成像(CTA)对第2肝门血管受侵的评价效能。方法回顾性分析由病理证实为HAE,并行自体肝移植的97例患者为研究对象,以术中探查结果作为金标准分析CTA检查对第2肝门血管受侵的诊断效能。结果97例中,肝左静脉受侵9支,肝中静脉受侵5支,肝右静脉受侵8支,肝后下腔静脉受侵4支,肝左、中静脉同时受侵16支,肝右、中静脉同时受侵19支,肝右、肝后下腔静脉同时受侵4支,3条肝静脉及肝后下腔静脉同时受侵32支;与手术结果比较,单个血管受侵诊断符合率84.62%,多个血管受侵诊断符合率63.51%,总符合率69%,差异有统计学意义(χ^(2)=10.217,P=0.03)。结论终末期HAE患者行自体肝移植术前应用CTA技术,能较为精确地掌握血管受侵情况。Objective To investigate the efficiency of computed tomography angiography(CTA)in evaluating vascular invasion of the second hepatic hilum in patients with end-stage hepatic alveolar echinococcosis(HAE)before autologous liver transplantation.Methods Ninety-seven patients with pathologically confirmed HAE who underwent autologous liver transplantation were analyzed retrospectively.Using the intraoperative detection results as the gold standard,the diagnostic efficacy of CTA in the diagnosis of vascular invasion of the second hepatic hilum was analyzed.Results In 97 patients,there were 9 branches of left hepatic vein,5 branches of middle hepatic vein,8 branches of right hepatic vein,4 branches of retrohepatic inferior vena cava,16 branches of left and middle hepatic veins,19 branches of right and middle hepatic veins,4 branch of right hepatic vein and retrohepatic inferior vena cava,and 32 branches of 3 hepatic veins and retrohepatic inferior vena cava were invaded.Compared with the results of operation,the diagnosis coincidence rate of single vascular invasion was 84.62%,and that of multiple vascular invasion was 63.51%.The total coincidence rate was 69%.The difference was statistically significant(χ^(2)=10.217,P=0.03).Conclusion CTA can accurately display the vascular invasion in patients with end-stage HAE before autologous liver transplantation.
