摘 要:目的探讨应用超声心动图在婴幼儿心脏横纹肌瘤(RM)诊断及治疗的价值,并分析头颅MRI影像特征。方法回顾性分析20例心脏RM患儿一般性临床资料、超声心动图及头颅MRI影像资料。分药物组、手术组进行疗效对比。结果治疗前左室射血分数(EF)值在2组之间差异有统计学意义,手术组EF值较低。治疗1个月手术组肿瘤最长径、肿瘤面积明显小于药物组。治疗6个月药物组肿瘤最长径、肿瘤面积较治疗前明显缩小,EF值较前增高。手术组肿瘤最长径、肿瘤面积较治疗前缩小或消失,但EF值差异无统计意义。20例患儿头颅MRI检查:结节性硬化8例,其中2例合并脑白质异常信号。结论超声心动图可诊断RM并动态评估其疗效。RM患儿行头颅MRI检查可明确有无结节性硬化症(TSC),头颅MRI检查有助于RM的临床诊断。Objective To investigate the value of echocardiography in the diagnosis and treatment,and to analyze the characteristics of brain MRI in infant cardiac rhabdomyoma(RM).Methods The data of clinical,echocardiography and brain MRI of 20 infants with cardiac RM treated were analyzed retrospectively.All infants were dividing into two groups,including drug group and operation group,to compare and analyze the therapeutic effect.Results There was a significant difference in left ventricular ejection fraction(EF)value between the two groups before treatment,and the EF value of the operation group was significantly lower than that of the drug group.After 1 month treatment,the longest diameter and tumor area of the operation group was significantly smaller than those of the drug group,respectively.Moreover,after 6 months treatment,the longest diameter and tumor area of the drug group were significantly reduced compared with those before treatment,and the EF value was obviously increased.The longest diameter and tumor area of the operation group was all reduced or disappeared compared with those before treatment,but there was no statistically significant difference in EF value.Of all 20 cases with brain MRI scanning,there were 8 cases with tuberous sclerosis,including 2 cases with abnormal white matter signal.Conclusion Echocardiography can be used to diagnose RM and evaluate dynamically the treatment efficacy.Tuberous sclerosis(TSC)in RM infants can be determined by brain MRI,improving diagnosis of RM in clinical practice.
