摘 要:目的:探究阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者多普勒超声下颈动脉斑块特征与鼾声响度、低氧血症及咽腔结构的相关性。方法:选择医院收治的97例OSAHS患者,依据鼾声响度将其分为轻度鼾声组(28例)、中度鼾声组(34例)和重度鼾声组(35例);依据最低动脉血氧饱和度(LSaO_(2))将其分为轻度低氧组(15例)、中度低氧组(33例)和重度低氧组(49例);依据咽腔结构分为轻度狭窄组(23例)、中度狭窄组(22例)和重度狭窄组(52例)。另选择同期在医院体检的68名健康体检者纳入健康对照组。检测所有受试者的心率、LSaO_(2)、咽腔内空气平均压力、鼾声响度、咽侧壁平均厚度及斑块检出率,分析OSAHS患者颈动脉斑块与鼾声响度、低氧血症及咽腔结构的相关性。结果:鼾声响度轻、中、重度3组,低氧血症轻、中、重度3组以及咽腔结构狭窄轻、中、重度3组斑块检出率均高于健康对照组(χ^(2)_(鼾声响度3组)2=4.658,χ^(2)=8.657,χ^(2)=11.325;x^(2)_(低氧血症3组)2=11.485,χ^(2)=11.859,χ^(2)=12.247;χ^(2)_(咽腔结构狭窄3组)=5.854,χ^(2)=10.524,χ^(2)=8.423;P<0.05)。经Pearson相关性分析,心率、鼾声响度与颈动脉斑块呈正相关(r=0.761,r=0.856;P<0.05);咽腔内空气平均压力、LSaO_(2)、咽侧壁平均厚度与颈动脉斑块呈负相关(r=-0.695,r=-0.761,r=-0.686;P<0.05)。结论:颈动脉斑块与鼾声响度、低氧血症和咽腔结构均与OSAHS发病有关,鼾声响度越大、LSaO_(2)越低及咽腔结构越狭窄,OSAHS患者出现颈动脉斑块的概率越高。Objective:To investigate the correlations between the characteristics of carotid plaque under the Doppler ultrasound and snoring loudness,hypoxemia and pharyngeal cavity structure in patients with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods:A total of 97 OSAHS patients who were treated in hospital were selected as the case group,which was further divided into mild snoring loudness group(28 cases),moderate snoring loudness group(34 cases)and severe snoring loudness group(35 cases)according to the degrees of snoring loudness,and was also further divided into mild hypoxia group(15 cases),moderate hypoxia group(33 cases)and severe hypoxia group(49 cases)according to lowest oxygen saturation(LSaO_(2)),and was also further divided into mild narrow group(23 cases),moderate narrow group(22 cases)and severe narrow group(52 cases)according to pharyngeal structure.A total of 68 healthy people who underwent physical examination in hospital during the same period were selected as the healthy control group.The heart rate,LSaO_(2),average air pressure in the pharyngeal cavity,the degree of snoring loudness,the average thickness of the lateral pharyngeal wall and the detection rate of plaque of all patients were detected.The correlations between carotid plaque and the degree of snoring loudness,hypoxemia and pharyngeal structure of OSAHS patients were analyzed.Results:The detection rates of the plaque of mild,moderate and severe snoring loudness groups,and those of mild,moderate and severe hypoxia groups,and those of mild,moderate and severe narrow groups were significantly higher than those of healthy control group(χ^(2)=4.658,χ^(2)=8.657,χ^(2)=11.325,χ^(2)=11.485,χ^(2)=11.859,χ^(2)=12.247,χ^(2)=5.854,χ^(2)=10.524,χ^(2)=8.423,P<0.05),respectively.Pearson correlation test showed that heart rate and the degree of snoring loudness were positively correlated with carotid plaque,and the average air pressure in pharyngeal cavity,LSaO_(2)and average thickness of lateral pharyngeal wall were negatively correlated
