晚期NSCLC患者围化疗期感染病原菌分布及感染危险因素分析

摘  要:目的分析晚期非小细胞肺癌(non-small cell lung carcinoma,NSCLC)患者围化疗期感染病原菌分布及感染危险因素。方法选取2018年6月至2021年6月浙江省台州医院收治的320例晚期化疗NSCLC患者作为调查对象,按照感染情况分为研究组(发生感染,n=65)和对照组(未感染,n=255)。统计晚期NSCLC患者围化疗期的感染率及病原菌分布,采用多因素Logistic回归分析探究晚期化疗NSCLC患者围化疗期感染的危险因素。结果共发出调查问卷320份,收回300份,其中晚期化疗NSCLC患者中有62例发生感染,感染率为20.67%。在62例感染患者中共检出病原菌80株,其中革兰阴性菌有48株,占比60.00%,革兰阳性菌26株,占比32.50%,真菌6株,占比7.5%。单因素分析显示晚期化疗NSCLC患者围化疗期发生感染与性别、临床分期、病理类型无关(P>0.05);晚期化疗NSCLC患者围化疗期发生感染与年龄、是否有侵入性操作、住院时间、是否使用抗生素、是否有合并疾病、血红蛋白、血清白蛋白、是否接受放疗有关(P<0.05)。多因素Logistic回归分析结果显示,年龄、是否有侵入性操作、住院时间、是否使用抗生素、是否有合并疾病、血红蛋白、血清白蛋白、是否接受放疗为影响晚期化疗NSCLC患者围化疗期感染的主要危险因素(P<0.05)。结论年龄≥65岁、有侵入性操作、住院时间≥20d、使用抗生素、血红蛋白<90g/L、血清白蛋白<30g/L、接受放疗为影响晚期化疗NSCLC患者围化疗期发生感染的危险因素,革兰阴性菌为主要病原菌。Objective To analyze the distribution of pathogenic bacteria and infection risk factors in patients with advanced chemotherapy non-small cell lung cancer(NSCLC)during the peri-chemotherapy period.Methods A total of 320 patients with advanced chemotherapy NSCLC who were admitted to Taizhou Hospital of Zhejiang Province from June 2018 to June 2021 were selected as the survey subjects and divided into study group(infection occurred,n=65)and control group(no infection,n=255)according to infection status.Counted the infection rate and pathogen distribution in the peri-chemotherapy period of patients with advanced NSCLC,and multi-factor Logistic regression analysis was used to explore the risk factors for peri-chemotherapy infection in patients with advanced chemotherapy NSCLC.Results A total of 320 questionnaires were sent out and 300 were returned.62 of the patients with advanced chemotherapy NSCLC developed infections,with an infection rate of 20.67%.A total of 80 pathogenic bacteria were detected in the 62 infected patients,of which 48 strains(60.00%)were Gram negative bacterium,26 strains(32.50%)were Gram positive bacterium and 6 strains(7.5%)were fungus.Univariate analysis showed that the occurrence of peri-chemotherapy infection in patients with advanced chemotherapy NSCLC was not associated with gender,clinical stage,or type of pathology(P>0.05);the occurrence of peri-chemotherapy infection in patients with advanced chemotherapy NSCLC was associated with age,presence of invasive procedures,length of hospital stay,use of antibiotics,presence of co-morbidities,haemoglobin,serum albumin,and receipt of radiotherapy(P<0.05).Multi-factor Logistic regression analysis showed that age,presence of invasive procedures,length of hospital stay,use of antibiotics,presence of co-morbidities,haemoglobin,serum albumin,and receipt of radiotherapy were the main risk factors affecting peri-chemotherapy infections in patients with advanced chemotherapy NSCLC(P<0.05).Conclusion Age≥65 years,invasive procedures,hospitalization time
